摘    要:目的 探讨病毒性脑炎患儿血糖变化和血气、电解质紊乱的临床特点及其临床意义。方法 将68例病毒性脑炎患儿按病情轻重分为轻症组与重症组,检测其血糖、血气分析和血清电解质水平,并对两组检测结果进行比较。结果 病毒性脑炎患儿高血糖、低钙血症、低钠血症、代谢性酸中毒的发生率分别为33.82%、44.12%、32.35%、33.82%,其中重症组血糖显著高于轻症组 (P<0.05),血钙、血钠、血PH值、剩余碱(BE)及碳酸氢根(HCO3-)均较轻症组显著降低(P<0.05或P<0.01)。结论 病毒性脑炎患儿存在内环境紊乱;血糖升高、血清钙和钠降低、代谢性酸中毒与病毒性脑炎病情关系密切,可作为反映病毒性脑炎病情轻重的客观指标。

Clinical Significance of the Internal Milieu Disorder in Children with Virus Encephalitis
Abstract:objective To explore the clinical features of the change of blood glucose, blood gas, electrolyte disturbances and their clinical significance in children with virus encephalitis. Methods 68 children with virus encephalitis were divided into mild cases group and severe cases group according to their pathogenetic condition. Blood glucose, blood gas analysis and electrolyte were measured in those cases and a comparative analysis was done between the two groups. Results The incidence of hyperglycosemia, hypocalcemia, hyponatremia and metabolic acidosis in children with virus encephalitis were respectively 33.82%, 44.12%, 32.35%, 33.82%,and compared with mild cases group, children with virus encephalitis in severe cases group had higher level blood glucose (P<0.05), lower lever blood calcium, blood natrium, power of hydrogen(PH), base excess(BE) and bicarbonate(HCO3-)(P<0.05 and P<0.01). Conclusion There is milieu disorder in children with virus encephalitis; Hyperglycosemia, hypocalcemia, hyponatremia and metabolic acidosis are associated intimately with pathogenetic condition, and they may be used as objective standards reflecting pathogenetic condition.
