肝硬化患者眼底血管改变的临床观察

目的观察肝硬化患者眼底血管改变,探讨肝硬化门脉高压与眼底血管分级的关系。方法肝硬化有、无食道静脉曲张破裂出血病人各52例,分别检查B超、肝纤维化指标和眼底血管等,并与正常人组进行对照,对检查结果进行统计分析。结果肝硬化患者眼底血管异常率明显高于正常人组,而并发食道静脉曲张破裂出血组眼底血管扩张Ⅱ级、Ⅲ级例数较无出血组显著增多。眼底血管扩张分级与门静脉及脾静脉内径、脾厚度、肝纤维化指标呈正相关。结论观察眼底血管扩张分级可作为了解肝硬化程度、门脉高压和预测胃底食道静脉曲张出血的一种指标。

THE CLINICAL OBSERVATION OF THE CHANGES OF EYE FUNDUS BLOOD VESSEL IN LIVER CIRRHOSIS PATIENTS

Objective To observe the changes of eye fundus blood vessel in liver cirrhosis patients in order to understand the relationship of eye fundus blood vessel and cirrhosis portal hypertension. Methods Cirrhosis patients with and without rupture of esophageal varices and hemorrhage,each group 52 patients, were examined with ultrasonic wave, blood vessel of eye fundus and the marker of hepatic fibrosis, with 52 normal persons as control group, the results were analyzed statistically. Results The abnormality rates of blood vessel of eye fundus in cirrhosis patients were significantly higher than normal person groups, in Ⅱ or Ⅲ grade Rates of blood vessel of eye fundus, the hemorrhage group was higher than the no hemorrhage group. The grade of blood vessel of eye fundus was positive correlation with portal vein and splenic v. bore, spleen depth, the markers of hepatic fibrosis. Conclusion Observation of the grade Rates of eye fundus blood vessel may be regarded as a marker of liver cirrhosis degree and portal vein pressure and predict the rupture of esophageal varices hemorrhage.
