摘    要:目的探讨冠状动脉粥样硬化性心脏病(冠心病)患者中乙醛脱氢酶2(ALDH2)基因多态性与高血压的关系。方法焦磷酸测序法分析57例冠心病患者ALDH2基因G487A多态性,并根据基因型分为两组:ALDH2基因GG型组(n=43)和GA/AA型组(n=14),比较两组间高血压的发病率、心血管病传统危险因素及治疗状态下收缩压、舒张压、脉压等指标的差异。结果 ALDH2基因GA/AA型组高血压的发病率(85.7%)明显高于GG型组(53.5%),差异有统计学意义(P<0.05)。两组之间收缩压与舒张压异无统计学意义(P>0.05),而脉压GA/AA组明显大于GG组,差异有统计学意义(P<0.01)。结论 ALDH2基因G487A多态性可能是高血压发病的遗传危险因素之一,其机制可能与影响内源性一氧化氮生成有关。其对硝酸甘油作用的影响可能增大老年冠心病患者的脉压。

Abstract:Objective To investigate the correlation between G487A variant of aldehyde dehydrogenase-2(ALDH2)gene polymorphisms and hypertension in patients with coronary heart disease.Methods Fifty-seven patients with coronary heart disease were enrolled in this study.The genotypes of ALDH2 were determined by pyrophosphate sequencing.The patients were divided into two groups according to the genotypes,namely GG group(n=43)and GA/AA group(n=14).The incidence of hypertension and the risk factors of cardiovascular disease and systolic/diastolic/pulse blood pressure,on treatment were assessed.Results The hypertension incidence of GA/AA group(85.7%)was significantly higher than that of GG-type group(53.5%),in the statistically significant manner(P= 0.05).The difference in systolic and diastolic blood pressure between the two groups were not statistically significant(P>0.05),while the pulse pressure of GA/AA groupwas significantly higher than that of GG group,in the statistically significant manner(P<0.01).Conclusion G487A variant of aldehyde dehydrogenase-2(ALDH2)gene polymorphism might be a risk factor for hypertension,one of the mechanisms related to endogenous NO synthesis.In addition,its effect on nitroglycerin might increase the pulse pressure in old patients with coronary heart disease.
