卵巢上皮性肿瘤组织中肺耐药蛋白的表达及与化疗耐药的关系

目的：探讨卵巢上皮性肿瘤肺耐药蛋白（LRP）表达的临床意义。方法：采用免疫组化法测定79例卵巢上皮性肿瘤（恶性54例、交界性5例、良性20例）和18例正常卵巢组织的LRP和P-gp表达，并进行相关临床因素分析。结果：⑴卵巢上皮性癌组织中LRP-Pgp表达阳性率分别为75.9%和38.9%。5例交界性肿瘤中LRP阳性4例，P-gp阳性2例。20例良性肿瘤和18例正常卵巢组织中分别有6例7例LRP表达阳性（30%和38.9%)，无1例P-gp阳性；⑵LRP的表达与腹水相关，与患者年龄、临床分期、病理类型、组织分级、肿瘤和残余瘤大小无关。P-gp的表达与上述临床特征无关；⑶LRP表达阳性和阴性的卵巢上皮癌患者化疗的有效率分别为50.0%和84.6%(P＜0.05)。P-gp表达阳性和阴性的患者化疗有效率分别为40.0%和69.7%（P＜0.05)；⑷LRP、P-gp表达阴性患者预后明显优于阳性者。结论：LRP是评价卵巢上皮性癌化疗耐药性和预后的可靠指标。

Expression of lung resistance protein and i ts relationship with drug resistance in epithelial ovarian tumor.

Objective:To expl ore the clinical significance of lung resistance protein (LRP) in ovarian epithelial tumors.Methods:The expressions of LRP and P-gp were examined by immunochemical methods in 54 ova rian epithelial carcinomas,15 borderline tumors,20 benign tumors and 18 normal o varian tissues.The relationships among the expression of two factors and clini cal pathologic features,resistance to chemotherapy and prognoses were analyzed.Results:1.The positive rates of LRP and P-gp in ovarian epitheli a l carcinoma were 75.9% and 38.9%,repectively.Of the five borderline tumors 4 we r e LRP-positive and 2 were P-gp-positive.6 benign tumors and 7 normal controls sh o wed LRP positive,while none of the ovarian benign tumors and normal control was found P-gp expression.2.The expression of LRP was associated with presence of ascites,no t with age,FIGO stage,histologic type,grade,residual tumor and the size of t umor.The expression of P-gp was not associated with any clinicopathologic features;3.The response rates to chemotherapy in patients with LRP positive and LRP negative were 50.0% and 84.6%,respectively(P＜0.05),while th at o f P-gp were 40.0% and 69.7% respectively(P＜0.05).4.The prognose s of LRP posit ive or P-gp positive patients are poorer than those of negative patients respect ively. Conclusions:LRP expression in ovarian epithelial carcionma is re lated to drug resistance and it is an useful prognostic factor.
