高频彩色多普勒超声诊断腮腺区肿块的临床价值

目的 探讨高频彩色多普勒超声(HCDU)对腮腺区肿块诊断及鉴别诊断的临床价值.方法 应用高频超声对49例患者腮腺肿瘤病变的边界、形态、内部回声及病变内有无钙化灶进行观察,应用彩色多普勒血流成像(CDFI)观察肿瘤的血流信号强度并分级,同时测量病变内血流频谱的RI值和Vmax,超声检查结果与手术病理结果对照分析.结果 49例患者中,HCDU对腮腺区肿块的检出率达到了100%,肿块的物理性质(囊实性)符合率87.50%,良性肿瘤的符合率为91.00%,恶性肿瘤符合率为81.25%.HCDU对恶性肿块的敏感性、特异性、诊断符合率分别为81.25%、91.00%、81.25%,淋巴结转移诊断符合率为83.33%.结论 高频彩色多普勒超声对腮腺肿块的定位定性具有重要的临床价值,能够提高腮腺恶性肿瘤诊断的正确率,是腮腺区肿块诊断的首选方法.

Clinical value of high frequency color Doppler ultrasonography in diagnosis of parotid tumors

Objective To evaluate the clinical value of high-frequency color Doppler ultrasonography(HCDU) in the diagnosis and differential diagnosis of parotid gland tumor.Methods 49 cases of parotid gland tumors underwent HCDU examination.The boundary,shape,interior echoes and calcific deposit dot of lesions were observed.The flow signal intensity of tumors were detected by color Doppler flow imaging(CDFI) and classified.The Vmax and RI of blood flow frequency spectrum inside the lesion were measured.The results were compared with the pathologic findings after operation.Results In 49 cases,the detectable rate of the focus was 100%,the diagnostic accuracy rates of the benign tumor,malignant tumor and the cyst were 91.00%,81.25% and 87.50%,respectively by using HCDU.In diagnosis of malignant parotid tumors,the sensitivity,the specificity and the accuracy of HCDU was 81.25 %,91.00% and 81.25% respectively.The accuracy of HCDU in diagnosis of cervical lymph node metastasis was 83.33%.Conclusion HCDU has significant clinical value in localizing and qualifying diagnosis of parotid diseases,and it can promote the accuracy diagnosis in malignant tumor of parotid gland.It is the best method in clinical diagnosis of parotid tumors
