原发性肝癌的放射介入治疗体会

摘    要:目的评价原发性肝癌经肝动脉化疗或栓塞的效果。方法对52例行肝动脉栓塞化疗的肝癌患者临床资料进行回顾性分析。结果本组52例患者全部行介入化疗,将化疗药物注人肿瘤供血血管局部进行化疗,提高了肿瘤病灶部位的药物浓度,化疗副反应小,大部分患者表现为胃肠道反应,如恶心、呕吐、厌食等,对症处理后均缓解,本组14例出现自细胞总数下降,使用升白药物后缓解。介入治疗后,33例患者自觉症状明显好转,肝区疼痛缓解,食欲改善。25例AFP下降或恢复正常。18例B超、CT提示肿瘤缩小50%以上。随访半年生存率4615%(24/52),1年生存率25.00%(13/52),2年生存率佰.46%(7/52),3年生存率5.77%(3/52)。结论肝动脉栓塞化疗具有微创、安全、高效的优点,是目前治疗肝癌的重要方法,具有重要的临床使用价值。

Abstract:Objective To evaluate the effect of chemotherapy or hepatic artery embolization in primary liver cancer. Methods The data of 52 patients with liver cancer reveived hepatic artery embolization were retrospectively analyzed. Results TACE was done. in all 52 patients. The chemotherapy drugs were injected into the blood vessels supplying the tumor to improve the drug concentration. The side effects of chemotherapy were small. The majority of patients presented with gastrointestinal reactions, such as nausea, vomiting, anorexia, etc., and the symptom remission after treatment. In 14 patients the number of white blood cells decreased, and increased after medication. After intervention, symptoms significantly improved in 33 patients, liver pain relief, appetite improved. AFP decreased or turned normal in 25 cases. Tumor shrinkage more than 50% in 18 cases with B-ultrasound, CT prompted. Follow-up six months survival rate was 46.15% (24/52), 1-year survival rate 25.00% (13/52), 2-year survival rate of 13.46% (7 / 52), 3-year survival rate of 5.77% (3 / 52).Conclusion Hepatic arterial chemoembolization with minimally invasive, safe and efficient advantages, is an important method of treatment of liver cancer.
