急诊介入治疗产后大出血的临床应用

目的探讨急诊经导管动脉栓塞术介入治疗产后大出血的应用价值。方法18例产后大出血患者，经seldinger行右股动脉穿刺插管，双侧髂内动脉造影，明确出血部位后，用明胶海绵颗粒、浆、条行子宫动脉栓塞术或髂内动脉栓塞术。结果本组所有产后大出血患者均在50～60min内完成栓塞术，手术时间平均468min，术后12例患者无再出血，4例患者阴道流血明显减少，仅有少量血性分泌物，2例患者阴道出血约20～50ml左右，出血3～7d自行停止。结论急诊动脉栓塞介入治疗产后子宫大出血相对于传统的止血方法效果好，具有创伤小，止血快，恢复快，并发症少，病死率低，并保留子宫和卵巢的生理功能。

The treatment of emergent interventional radiology in postpartum hemorrhage

Objective To evaluate the outcome of transcatheter arterial embolization （TAE） in postpartum hemorrhage. Methods Uterine arterial embolization（UAE） or internal iliac artery embolization（IIAE） was performed with gelfoam in 18 patients under the monitoring of DSA to show the bleeding sites. Results The bleeding was stopped in all patients with no serious complications. Conclusion TAE is an effective method for treating postpartum hemorrhage for its safety, easy performance and instant stoppage of bleeding, not only patients＇life but also their uterine are saved.
