单层上皮细胞角蛋白在声门上型喉癌中的表达

目的 探讨角蛋白(cytokeratin,CK)19,CK18在声门上型喉癌中的表达及其临床意义。方法 用免疫组化法观察60例声门上型喉癌术后标本CK19,CK18的表达。结果①60例声门上型喉癌组CK19表达阳性53例,阳性率88.3％,而CK18呈阴性表达;②60例声门上型喉癌CK19表达与病理分级有关:中分化组与低分化组(P=0.019),高分化组与低分化组(P=0.003)之间的差异显著,而与TNM分期无关(P=0.186);③60例声门上型喉癌CK19表达在有淋巴结转移组明显高于无淋巴结转移组(P=0.019);④60例声门上型喉癌CK19表达与生存时间无关(P=0.438)。结论60例声门上型喉癌CK19表达与病理分级及淋巴结转移有关,而与TNM分期及生存时间无关。

Significance of the expressions of cytokeratin protein in supraglottic laryngeal carcinoma

Objective To explore the significance of the cytokeratin-18 protein (CK - 18) and cytokeratin -19 protein (CK -19) in patients with supraglottic laryngeal carcinoma. Methods The expressions of CK - 18 and CK - 19 were detected by immunohistochemical method in specimens of 60 patients with supraglottic squamous cell carcinoma from 1995 to 1996. There were 37 males and 23 females,ranging in age from 36 to 79 years old. The TNM stage of those patients were T1N0 in 2,T2N0 in 14, T2N1 in 5,T1N2 in 5,T2N2 in 5,T3Noin 7,T3N1, in 8,T3N2in 8,T3N3 in 1, T4N0 in 3, T4N1 in 4 and T4N2 in 3. The pathological grades were 22 in 1,20 in *,18 in * respectively. Results The expression positive rate was 88.3 % (53 / 60) in CK -19 and 0.0 % (0 / 60) in CK -18 respectively. The expression of CK -19 had significant differences between the pathologic grades * and III (P = 0.019), I and * (P = 0.003),but did not correspond with clinical stage TNM (P = 0.186). The expressions of CK - 19 in the supraglottic carcinoma patients with metastasis lymph node group were higher than that in the non-metastasis group (P = 0.019). The expressions of CK - 19 did not correspond with survival times (P = 0.4538). Conclusion The expressions of CK - 19 in 60 patients with supraglottic laryngeal carcinoma correspond with the pathologic grades and metastasis lymph node, but do not correspond with clinical stages and survival time.
