罗哌卡因复合液对术后膀胱痉挛镇痛的研究

摘    要:目的探讨硬膜外持续榆注0.2%罗哌卡因、吗啡复合液对前列腺摘除术后膀胱痉挛镇痛的效果及其副作用。方法对40例AsAI~Ⅲ级,前列腺摘除术的患者随机分成A、B2组,A组患者术毕经硬外管注入0.2%罗哌卡因5ml,内含吗啡1.5-2mg作为负荷量,然后硬外管接美国Baxter镇痛泵,榆注0.2%罗哌卡因48ml,内含吗啡4mg、胃复安20mg,以1ml/h速度持续榆注;B组患者术毕经硬外管注入吗啡1.5-2mg作为负荷量,然后硬外管接美国Baxter镇痛泵,榆注0.9%N.S 48ml内含吗啡4mg、胃复安20mg,1ml/h速度持续输注。分别观察各组伤口疼痛VAS评分,膀胱痉挛性疼痛VAS评分,双下肢运动阻滞评分,同时观察恶心呕吐、皮肤瘙痒和呼吸抑制等发生率。结果2组对患者伤口镇痛效果好,VAS评分无差异,2组双下肢运动阻滞程度无明显差异,恶心、皮肤瘙痒每组各1例,2组均未出现呼吸抑制及呕吐。对膀胱痉挛性疼痛的抑制A组优于B组,2组有非常显著性差异(P〈0.01)。结论0.2%罗哌卡因、吗啡复合液用于前列腺摘除术后镇痛效果好,能明显抑制术后膀胱痉挛性疼痛,且副作用少。

Abstract:Objective To evaluate the application of Micro-Stryker Instruments combined with cystoscope and self-retaining laryngoscope for the resection of vocal cord polyp.Methods Vocal cord polypectomy through Micro-Stryker Instruments combined with cystoscope and self-retaining laryngoscope were performed on 96 patients. Results All patients were followed up for three months. Of them, 87.5% were complete cured , 11.46% partial improved, and only 1.04% showed no effect. Conclusions Micro-Stryker Instruments combined with cystoscope and self-retaining laryngoscope was a valuable approach for the resection of vocal cord polyp,which was easy to operation, simple, safe, highly effective, and therefore it was suitable for broadly clinical application.
