脑干肿瘤的显微手术治疗

目的探讨脑干肿瘤手术治疗的适应证,手术方法和疗效。方法回顾我院1999年7月～2004年7月,22例脑干肿瘤显微手术治疗的资料,从临床表现、影像学检查、病理类型、手术适应证、手术方法和术后并发症等方面予以分析。结果2例中脑肿瘤、8例桥脑肿瘤、2例延髓肿瘤,5例延脊型肿瘤,术后症状明显改善。2例延髓肿瘤、1例中桥脑肿瘤、1例桥延脑肿瘤术出现呼吸衰竭,其中1例好转,2例死亡、2例自动出院;1例桥脑肿瘤术后第一天出现肾功能衰竭,昏迷,自动出院。结论正确选择手术适应证,个性化的手术方法,术后并发症的预防和及时处理是脑干肿瘤手术成功的关键。

Microsurgical treatment of brainstem tumors

Objective To explore the clinical indication?methods and curative effective of surgical treatment for the brainstem tumors.Methods The cases of 22 patients (9 males and 13 females,aged from 6 to 69 years) with lesions in brainstem were retrospectively reviewed,during the last five years(July,1999 to July,2004).The brainstem lesions were distributed as follows;pons in 9 cases,medulla in 4,midbain in 2,pontomesencephalic junction in one and medullarto Myelen cephalon junction in one.The clinical histories,patholog records ,surgical indications,operative reports and complications of every case were evaluated.All the patients were underwent microsurgical treatment.Results Of all these,17 were the same or better after microsurgical intervention,3 were worse,and 2 were died.Conclusion With carefull patient selection,the lesions in bainstem can be treatemted by microsurgical successfully,to prolong the time of lives of patients is necessary and important,while improve the quality of lives of patients is our purpose.
