原发性纵隔肿瘤67例诊断及胸腔镜手术治疗体会

目的总结原发性纵隔肿瘤的临床诊断经验，探讨电视辅助胸腔镜手术在纵隔肿瘤治疗上的临床应用体会，提高原发性纵隔肿瘤外科诊治水平。方法对67例行手术治疗的纵隔肿瘤进行诊断和手术方法分析，并进行归纳总结。结果单纯胸腔镜手术切除63例，胸腔镜辅助颈部小切口手术1例，胸腔镜手术中转开胸1例，胸腔镜手术探查活检2例，术后1例出现肌无力危象，其余恢复顺利，无围手术期死亡发生。结论电视辅助胸腔镜手术适用于大部分纵隔肿瘤，具有创伤小、痛苦轻、恢复快、安全可靠的优势。

Diagnosis and video-assisted thoracic surgery treatment experience of 67 patients with primary mediastinal tumors

Objective To summarize the experience of clinical diagnosis to primary medias- tinal tumors and investigate the clinical application of video-assisted thoracoscopic surgery in the treatment of mediastinal tumors to improve the surgical diagnosis and treatment of primary medi- astinal tumor. Methods The diagnostic and surgical methods of 67 patients who were mediasti- nal tumors were analyzed and summarized. Results A total of 63 tumors were resected by video assisted thoracic surgery alone, one tumor were resected by video-assisted mini-thoracotomy, one of the operations were converted to thoracotomy, two patients only to surgical exploration and biopsies. After operation,one patient occurred myasthenia crisis, the others recovered uneventfully, no preoperative death. Conclusion Video-assisted thoracic surgery applies to most of the mediastinal tumors with the advantages of less trauma and pain, quick recovery, safety and reliable.
