CT与腰穿对原发性蛛网膜下腔出血诊断价值的比较

目的：比较CT与腰穿两种方法诊断原发性蛛网膜下腔出血(原发性SAH)的价值。方法：分析60例原发性SAH患者CT与腰穿检查结果。结果：60例患者中38例CT阳性，总阳性率为63.33％。不同时间内CT阳性率分别为：1天内86.67％，1～3天为70％，3天内为82.5％，4～6天为33.33％，7～12天为18.18％，4～12天为25％，3天内的CT阳性率高于4～12天(P＜0.01)。原发性SAH12天内，腰穿阳性率为100％，腰穿阳性率高于CT(P＜0.01)。结论：CT的阳性率无腰穿高，但其为无创性检查，可排除继发性SAH，临床怀疑原发性SAH时，应首选并及时行CT检查。CT阴性者不能排除原发性SAH，应做腰穿检查。

The estimation of CT and Iumber puncture in the diagnosis of primary subarachnoid hemorrhage

Objective:to estimate the value of CT and lumbar puncture in diagnosis of primary SAH Methods:Sixty patients with primary SAh were examinated by CT and lumbar puncture Results:The result showed positive in 38 of 60 patients by CT,Total positive rate was 63 33% The positive rates of CT were 86 67% within one day,70% from first to 3rd days,82 5% in third day,33 33% in 4th to 6th days,and 18 18% in 7th to 12th days,25% in 4th to 12th days The positive rates in three days was higher than that in 4th to 12th days The positive rate of lumbar puncture was 100% and was higher than that of CT Conclusion:These results indicate:The positive rate of CT is lower than that of lumbar puncture,but CT is noinvasive and assisitance to exclude from secondary SAH When primary SAH is in doubt,CT shows negative and primary SAH cannot be excluded,lumbar puncture should be taken
