摘    要:目的:观察大量输血前后患者凝血功能和血小板变化,为临床输血治疗提供依据。方法:筛选各类24h内大量输注悬浮红细胞血患者87例,临床检测记录患者输血前后24h内的凝血功能和血小板数值,进行统计学比较。结果:在大量输注悬浮红细胞液后,所有患者的血小板(PLT)和纤维蛋白原(FIB)数值均较输血前降低,而活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)和凝血酶时间(TT)与输血前比较明显延长,患者凝血功能和血小板输血前后24h检测数值比较,差异具有统计学意义(P〈0.05)。结论:大量输注悬浮红细胞液后,会导致患者凝血功能以及血小板降低,临床大输血患者应密切观察检测凝血功能和血小板情况,危重患者可使用血栓弹力图动态记录患者凝血情况,必要时补充血小板、血浆等成分血,降低或避免因血小板和凝血功能异常引起的出血风险。

Abstract:Objective:To observe changes in blood coagulation function and platelet before and after undergoing massive blood transfusion.Methods:87cases that had received suspended red blood cells transfusion within 24 hour were selected.blood coagulation function and platelet numerical before and after the massive blood transfusion were detected and compared.Results:After undergoing the infusion therapy,platelet(PLT)and fibrinogen(FIB)values reduced in all patients,and the live clotting enzyme activation part time(APTT)、prothrombin time(PT)and thrombin time(TT)were obvious extended.Blood coagulation function and platelet values differed significantly before and 24 hafter transfusion therapy(P〈0.05).Conclusion:After massive infusion of suspension red cells,blood coagulation function and platelets reduce significantly indicating it needs close monitor.Necessary measures such as detecting blood platelet,plasma,should be taken to avoid bleeding.
