DEC-MRI在乳腺疾病诊断中的应用价值

目的：探讨磁共振动态增强成像（DCE-MRI）在乳腺疾病诊断中的应用价值。方法：选取本院经病理证实为乳腺疾病患者60例,其中良性病灶26例,恶生病灶34例,本组患者入院均行DCE-MRI、传统超声及乳腺钼靶检查,对比3种检查结果与病理检查结果的符合性。结果：DCE-MRI诊断准确符合率优于传统超声及乳腺钼靶检查,差异具有统计学意义（P〈0.05）;DCE-MRI对乳腺良恶生疾病诊断的敏感性、特异性均优于钼靶检查及超声检查,差异具有统计学意义（P〈0.05）;钼靶检查及超声检查敏感性、特异性、阳性预测值、阴性预测值比较,差异无统计学意（P〉0.05）。结论：DCE-MRI可满意显示乳腺良恶性病变的形态学特征和增强的血液动力学特点,对提高乳腺病变良、恶性病变鉴别诊断的准确率有不可替代的作用,应做为首选检查方法。

Significance of DEC - MRI in breast disease diagnosis

Objective：To investigate the application significance of Dynamic Contrast Enhanced MRI（DCE MRI）value in the diagnosis of breast disease.Methods：A total of 60 patients with mammary gland disease admitted in our hospital and underwent DCE MRI were selected,traditional ultrasound and mammary gland molybdenum target inspection,including 26 cases of benign lesions and 34 cases of malignant lesions confirmed by pathology examination in this study,conformity of the test results and pathology results were compared.Results：Diagnostic accuracy rate of DCE MRI,mammary gland molybdenum target inspection was better than traditional breast ultrasound detection.Diagnosis coincidence rate,diagnostic sensitivity and specificity of DCE MRI was significantly higher than that of mammary gland molybdenum target inspection and traditional breast ultrasound（P〈0.05）.There was statistical difference of sensitivity,specificity,positive predictive value,negative predictive value between mammary gland molybdenum target inspection and traditional breast ultrasound（P〈0.05）.Conclusion：DEC-MRI can ideally demonstrate the morphology and hemodynamics enhancement characteristic in breast malignant lesions,improve the differential diagnosis accuracy of benign and malignant breast lesions as the preferred method.
