肾上腺肿瘤的CT诊断及其评价

目的：回顾分析肾上腺肿瘤的影像学检查，进一步探讨CT对肾上腺肿瘤的诊断价值。材料与方法：55例经手术病理及临床随访证实的肾上腺肿瘤的CT特征并与B超结果对照分析。结果：无功能性腺瘤的平均长径大于6cm，边界多清；功能性皮质腺瘤多较小，11／12例见同侧正常肾上腺组织；嗜铬细胞瘤多密度不均匀，边界清；髓脂瘤CT值多为负值；皮质腺癌直径多大于6cm，密度不均匀，边界不清；转移瘤8／10例直径小于3cm，多伴原发肿瘤及其它脏器转移瘤的存在。肾上腺肿瘤CT定性符合率85．5％，定位符合率96．4％。结论：CT对肾上腺肿瘤的定位及定性诊断、各肿瘤之间的鉴别诊断特性较其它影像学检查优越。

The Diagnostic Value of CT Imaging to Adrenal Neoplasms

Purpose: To review the imaging studies of adrenal neoplasms retrospectively and to evaluate the diagnostic efficacy of CT in this field. Materials and Methods: The CT manifestations of 55 cases, proved surgically, pathoogically and by clinical folowup, were analyzed retrospectively, also compared with ultrasound results. Results: The average diameter of non functional adrenal adenoma is more then 6cm, with clear margin; Functional cortex adenoma is smaller usually, 11/12 of these cases with normal adrenal tissue surrounding; The CT attenuation value of myelolipoma usually reveals negative; The diameter of adrenal cortical carcinoma is commonly larger than 6cm, reveals heterogeneous with indistinct margin; The diameter of 8/10 cases with metastases is usual smaller than 3cm, and original tumours or metastases in other organ could be found at same times. The accuracy rate of differentiation and localization is 85. 5 % and 96. 4 % respectively. Conclusion: CT is more useful examination in differentiation and localization of adrenal neoplasms.
