两种全耳再造术并发症情况的临床分析

目的：比较Tanzer-Brent分期法（分期法）全耳再造术和乳突皮肤完全扩张法（扩张法）全耳再造术两种术式并发症的好发种类。方法：对47例小耳畸形患者行分期法全耳再造术，14例小耳畸形患者行扩张法全耳再造术，统计两种不同术式并发症的种类。结果：行分期法全耳再造术患者出现并发症9例（19％，9／47），其中耳软骨支架外露3例，感染l例，颅耳角狭窄3例，耳廓小于健侧耳2例；行扩张法全耳再造术出现并发症7例（50％，7／14），血肿3例，耳软骨支架外露2例，扩张期外露2例，颅耳角狭窄1例。分期法全耳再造术的并发症发生率明显低于扩张法（P〈0．05）。结论：两种术式比较，分期法并发症较少，血肿形成、耳软骨支架外露、感染、耳颅角狭窄、再造耳小于健侧耳等是全耳再造术常见的并发症。

Analysis on complications after two different surgical methods of total auricular reconstruction

Objective： To compare the types and characteristics of complications after two different surgical methods of total auricular reconstruction, which were stage method and mastoid skin expansion method respectively.. Methods： Forty seven patients accepted total auricular reconstruction by stage method, and 14 by expansion method. Retrospective study was conducted to analyze the complications occured after two different surgical methods. Results： Nine （19 % ） cases accepting total auricular reconstruction by stage method occurred complications, including 3 cases of framework exposure, 1 case of infection, 3 cases of narrow cranioauricular angle and 2 cases of smaller auricle compared with the healthy side. Seven （50 % ） cases accepting total auricular reconstruction by expansion method occurred complications, including 3 cases of haematoma, 2 cases of framework exposure, 2 cases of framework exposure during dilation stage and 1 case of narrow cranioauricular angle. Fewer complications occured ater stage method was adopted than expansion method. Conclusion： Stage method has lower complications occurrence in total auricular reconstruction.
