2型糖尿病对冠心病粥样硬化斑块影响的CTA评价

摘    要:目的通过64排MSCTA对2型糖尿痛合并冠状动脉病变(C0ronary arlery disease,CAD)及非糖尿病CAD患者冠脉病变进行比较分析,探讨2型糖尿病合并CAD冠脉成像斑块类型的特点。材料与方法对35例2一型糖尿病ChD患者(DM组),41例非糖尿病chD患者(NDM组)行64排MSCT血管成像及工作站后处理,对两组冠状动脉粥样硬化病变分布节段、斑块的类型进行统计学分析与比较。结果1。DM组76支病变血管,人均2.17±1.1走,共87个冠脉班块节段,人均2.48±1.5节段:NDM组83支病变血管,人均1.88±1.O史,89个冠脉斑块节段,人均2.02±1.05节段:2.DM组混合斑块及冠脉弥漫病变发生较NDM组明显增高,DM纽病变管腔中度以上狭窄高于NDM组,差异有统计学意义(P〈0.05)。3.二组间钙化斑块、非钙化兜块在冠脉分布上,病变支数上无统计学差异(P〉0.05)。结论2型糖尿病合并冠心痛粥样硬化斑块的特点:混合斑块的发生率较非糖尿病冠状动脉病变高,且管腔狭窄较非糖尿病严重。

Abstract:Objective To assess the characteristic of atherosclerosis plaque in patients who had both type 2 diabetic and Coronary artery disease(CAD) by comparing with the ones who had CAD simply by using coronary CT angiography (CTA). Materials and method Study was conducted from I st January 2010 to 20th July 2011 with 79 CAD patients, 35 in DM group( 20 male, 15 female;mean age,68iA 10years) and 41 in non-DM group. Their CTAdata were reviewed, and the extent, distribution and types of plaques were evaluated and cnmpared between DM group and Non DM group(NDM). Result First,76 vascular lesion (2.17 ± 1.6 per capita) and 87 coronary segments (2.48 + 1.5 per capita) in total were found having plaques of CAD in DM group: In NDM group 83 vascular lesion( 1.88 ±1.0 per capital and 89 coronary segments (2.02 ± 1.05 per capita) in total were found having plaques. Secondly, mixed plaques was significantly higher in the DM group than in the NDM group, and more severe stenosis were found in DM group than in the NDM group, the difference was statistically significant (P 〈0.05) .No statistical difference s(P〉0.05) were found in the distribution and count of plaques between the two groups. Conclusion The more mixed plaque and severe stenosis are the characteristics of coronary artery disease in symptomatic type 2 diabetic patients, relative to the non-diabetic CAD by CT angiography.
