MAITL、CARMA1及BCL10蛋白在MALT淋巴瘤和结外弥漫大B细胞淋巴瘤中的表达及其意义

摘    要:目的 了解MALT1、BCL10、CARMA1蛋白分子在结外弥漫大B细胞淋巴瘤(Ex-DLBCL)及MALT淋巴瘤中的表达及表达的相互关系,探讨其表达对Ex-DLBCL及MALT淋巴瘤的影响.方法 通过常规苏木素伊红染色观察肿瘤的组织学特征,免疫组织化学染色检查对肿瘤细胞的免疫表型进行病理筛选,再用免疫组化染色检测CARMA1、BCL10、MALT1蛋白和Ki67的表达.采用相应的统计方法进行统计学分析.结果 蛋白表达总阳性率:在淋巴瘤中CARMA1 62.8%,MALT1 73.0%,BCL10 69.2%,表达水平明显高于反应性增生淋巴结(依次为13.3%,20%,13.3%),P值均为0.001.MALT淋巴瘤和Ex-DLBCL病例比较:CARMA1 (60.9%:64.8%)和MALT1(70.7%:75.6%)蛋白表达差异均无显著性,但BCL10表达(58.5%:81.0%)差异有统计学意义.CARMA1、BCL10和MALT1蛋白两两比较表现出明显的相关关系;三种蛋白不同的表达情况与淋巴瘤类型有相关关系(P=0.022),蛋白分子表达种类越多,Ex-DLBCL病例越多.CARMA1和BCL10阳性表达病例肿瘤细胞Ki67≥30%的病例明显多于相应的阴性表达病例(P=0.001),而MALT1阳性和阴性表达病例之间比较无类似的差异.结论 MALT和Ex-DLBCL肿瘤细胞高表达CARMA1、MALT1和BCL10,三种蛋白分子之间两两表达具有相关性,CBM三联复合物形成可能通过使NFκB的异常持续活化上调促细胞增殖的相关基因表达从而参与MALT淋巴瘤的演进过程.

Abstract:Objective To detect the expression of CARMA1,BCL10 and MALT1 in MALToma and Extra diffuse large B cell lymphoma(Ex-DLBCL) and their correlation,investigate effects of the expression on lymphomas. Methods The cases were filtrated by HE and immunohistochemical(IHC) stains.Expression of CARMA1,BCL10,MALT1 and Ki67 were assayed by IHC stain.Statistical analysis was done. Results Total positive rate in lymphomas shown that CARMA1 was 62.8%,MALT1 was 73.0%,and BCL10 was 69.2%.The expression were higher than those in reactive hyperplasia of lymph node(13.3%,20%,13.3% orderly),both of P value were 0.001.Comparing MALToma with Ex-DLBCL,the expression of CARMA1(MALToma vs Ex-DLBCL:60.9%:64.8%) and MALT1(MALToma vs Ex-DLBCL: 70.7%:75.6%) had no statistical difference,however,the difference in expression of BCL10 was observed(MALToma vs Ex-DLBCL: 58.5%:81.0%).The expression of three proteins showed correlation.The expression of three molecules showed correlation with type of lymphoma.More molecule types of expression,more cases of Ex-DLBCL.In contrast with the cases with negative expression of CARMA1 and BCL10,those with positive expression had more cases with Ki67≥30%(P=0.001).Whereas it was not observed between the cases with positive and negative in expression of MALT1. Conclusion CARMA1,MALT1 and BCL10 were expressed highly in MALToma and Ex-DLBCL.The expression of three proteins has correlation in two lymphomas.CBM complex maybe up regulate expression of genes associated with cellular proliferation by promoting activation of NFκB.It may be one of important mechanism of progress of lymphomas.
