膝骨关节炎患者血清MMP-3、TIMP-1水平变化及相关性研究

目的观察膝骨关节炎(KOA)患者血清基质金属蛋白酶-3(MMP-3)、基质金属蛋白酶组织抑制物-1(TIMP-1)变化并进行相关性研究。方法采用酶联免疫吸附法(ELISA)对60例KOA患者(KOA组)及30例正常对照组血清MMP-3、TIMP-1、白细胞介素-1β(IL-1β)、转化生长因子-β(TGF-β)进行检测;采用流式细胞术检测外周血CD4+CD25+CD127low/-调节性T细胞(Treg)比例;依据KOA严重程度指数(LequesneMG)评分标准将60例KOA患者分为轻、中、重3组,并以60岁为界限将KOA患者分为<60岁组(26例)、≥60岁组(34例),分析KOA患者血清MMP-3、TIMP-1变化及与LequesneMG积分、症状量化积分、国际普适生活质量量表(SF-36)积分、焦虑自评量表(SAS)积分、抑郁自评量表(SDS)积分、红细胞沉降率(ESR)、超敏C-反应蛋白(hs-CRP)、IL-1β、TGF-β、CD4+CD25+CD127low/-Treg的相关性。结果①与正常对照组比较,KOA组血清MMP-3水平显著升高,TIMP-1水平显著降低(P<0.05)。②重度组血清MMP-3、TIMP-1水平显著高于轻度组及中度组,中度组显著高于轻度组(P<0.05)。③与<60岁组比较,≥60岁组血清MMP-3、TIMP-1显著升高(P<0.05)。④相关性分析显示KOA患者血清MMP-3、TIMP-1与LequesneMG积分、SAS积分、ESR、hs-CRP、IL-1β、TGF-β呈正相关,与SF-36各维度积分呈负相关,MMP-3与CD4+CD25+CD127low/-Treg呈负相关(P<0.05)。结论 KOA患者血清MMP-3显著升高,TIMP-1显著降低,其表达失调可能参与KOA发病过程。

The changes and correlations of MMP-3,TIMP-1 in knee osteoarthritis patients

Objective To observe the change of serum MMP-3,TIMP-1 in the knee KOA patients,and to study the correlation.Methods The serum levels of MMP-3,TIMP-1,IL-1β,TGF-β were detected by enzyme-linked immunosorbent assay(ELISA);CD4+CD25+CD127low/-Treg of peripheral blood was detected by flow cytometry;60 cases of KOA patients were divided into three groups of mild,medium,severe,based on the Lequesne MG;and with 60-year-old as a dividing line,l60 cases of KOA patients were divided into two groups,<60 years old group(26 cases),≥ 60 years old group(34 cases),and the changes of MMP-3,TIMP-1 of serum in the KOA and the relationship of the indexes in KOA patients were analysed.Results ① Compared with the control group,the levels of MMP-3 were significantly increased,TIMP-1 levels were significantly decreased in KOA patients(P<0.05).② MMP-3,TIMP-1 in severe group were significantly higher than those of the mild group and moderate group.The levels of serum MMP-3,TIMP-1 in moderate group were significantly higher than those of the mild group(P< 0.05).③ Compared with under 60 years old group,MMP-3,TIMP-1 were significantly higher than those of above 60 years old(P<0.05).④ Correlation analysis showed that the KOA serum MMP-3,TIMP-1 and LequesneMG integration,SAS points,ESR,high-sensitivity C-reactive protein,IL-1β,TGF-β were positively correlated with all SF-36 dimensions of integration,while CD4+CD25+CD127low/-Treg was negatively correlative(P<0.05).Conclusion That TIMP-1 decreases and MMP-3 increases in the serum of KOA patients significantly,its expression disorders may be involved in the process of KOA.
