重型颅脑损伤合并肺部感染患者的胸部影像学及病原学分析

目的通过总结分析重型颅脑损伤合并肺部感染患者的胸部影像学特点的临床和影像学特点,进一步加深对本病的认识,以减少漏诊和误诊。方法回顾分析40例重型颅脑损伤合并肺部感染患者的临床表现、胸部影像学特点,总结治疗体会。结果重型颅脑损伤合并肺部感染患者早期临床表现不典型,除呼吸系统症状和体征外,常有其他非特异性表现,在痰细菌培养阳性者中革兰阴性菌检出率高,影像学上以支气管肺炎为主,并发症发生率高。不同病原菌感染胸部影像学表现：病变部位、性质、伴随征象均不具有特异性。结论重型颅脑损伤合并肺部感染患者临床表现及胸部影像学不典型,容易漏诊、误诊。加强对此病的认识能提高重型颅脑损伤合并肺部感染患者的早期诊断率和抢救成功率。

Clinical analysis and Chest Radiographic Characteristics of pulmonary infection in patients with severe craniocerebral injury

Objective Through analysis of pulmonary infection in patients with severe craniocerebral injury with chest imaging characteristics of clinical and radiographic features,further deepen the understanding of the disease,in order to reduce missed diagnosis and misiagnosis.Methods To take a retrospective analysis on the clinical data of 40 patients of pulmonary infection with severe craniocerebral injury clinical manifestations, chest imaging characteristics,summary treatment experience.Results Severe craniocerebral injury with pulmonary infection in patients with early clinical manifestation is not typical,in addition to the respiratory system symptoms and signs,often have other nonspecific manifestations,in sputum bacterial culture was bacterium of negative of orchid detection rate is high,on the imaging is given priority to with bronchial pneumonia,higher incidence of complications.Pathological changes of different pathogens infection chest imaging findings：to be specific,nature,accompanied by signs are not.Conclusion]Severe craniocerebral injury with pulmonary infection in patients with atypical clinical manifestations and chest radiographic,easily missed diagnosis and misdiagnosis. Strengthen the awareness of the disease can improve diagnostic and rescue success rate
