摘    要:目的:观察伴中央中颞区棘波的儿童良性癫痫(BECT)长期控制中发作间期单光子发射断层扫描(SPECT)异常灌注灶恢复情况和V—EEG的变化。方法:观察25例BECT患儿经抗癫痫药治疗前后发作间期SPECT显像与长程EEG及影像学的变化。结果:控制发作时间平均30个月,用药前后SPECT、两次比较:用药后SPECT正常增加10例(67%,P〈0.05);低灌注减少1例(33%)和高灌注减少9例(53%),单灶减少9例(64%),多灶减少1例(17%)。EEG比较:用药后正常增加7例(28%,7/25;P〈0.05),CT和(或)MRI检查均无异常。结论:抗癫痫药物可改善BECT的异常灌注灶和减少痫样放电,部分异常灌注灶经治疗可修复。

Changes of SPECT abnormal perfusinon loci in paleints with benign epilepsy of chidren with centromidtemporal spikes

Abstract:Objective:To explore dynamic changes of interictcal SPECT abnormal perfusion fQci and V-EEG in patients with benign epilepsy of children centromidtemporal spikes(BECT) in long-term control of antiepileptic drugs(AEDs). Methods: Twenty-five patients with BECT were selected with AEDs treatment of long term control. Before and after interictcal SPECT ,long-term V-EEG and imaging examination were finished in the patients. Results:The average time of the whole control was 30 months. With twice comparison:the normal SPECT increased in 10 case(67.0%,P〈0.05) ; respectively, hypoperfusion loci was reduced in 1 case(33%) and hyperperfusion loci decreased in 9 case(53%) , single foci and polyfoci decreased respectively in 9 case and 1case(64.0% and 17.0%). With twice comparison of the nomal V-EEG was increased in 7 cases (28.0 %, 7/25, P〈0.05). The CT/MRI was normal . Conclusions: The AEDs can affecte abnormal perfusion foci of SPE.CT and epileptic discharges of EEG in the pateints with BECT and part of abnormal perfusion foci could be repaired.
