一期人工植骨内固定治疗关节内压缩性跟骨骨折的疗效观察

摘    要:目的:探讨一期人工植骨内固定治疗关节内压缩性跟骨骨折的临床疗效。方法:对2011-01-2011-12我科收治的120例关节内跟骨骨折行一期人工骨植骨内固定手术治疗的病例进行随访观察,按Maryland足部评分系统评级最终疗效。结果:120例患肢均得到随访,随访时间6~18个月(平均11.8个月)。术后发现2例切口部分裂开,3例切口边缘部分坏死,1例伤口反复渗出,对症处理后均恢复正常;2例距下关节炎、3例关节僵硬,3例跟骨骨刺影响行走;未发现骨折的再移位、关节面再塌陷及人工骨排异反应。应用Maryland足部评分系统对患足功能进行评分:优78例,良34例,可8例,优良率为93.3%。结论:一期人工骨植骨内固定治疗关节内压缩性根骨骨折可取得良好预后。

Abstract:Objective:To investigate the clinical effect of calcaneal fractures with joint compression treated with internal fixation combined primary artificial bone grafting. Method: From January 2011 to December 2011,120 pa- tients with calcaneal fracture and joint compression were treated by internal fixation combined primary artificial bone grafting. Thefinal efficiency was evaluated by with Maryland clinical function o. Result: All patients had been followede for 6 to 18 months (an average of 11.8 months). We found 2 cases of cut part of dehiscence,3 cases of partial necrosis of the edge of the incision, 1 case of wound repeatedly exudation, and alI returned normal after symptomatic treatment. And 2 cases of subtalar joint inflammationg,3 cases of anchylosis, 3 cases of calcaneal spur impact of walking were found, without significant re--shift of the fracture, the articular surface re--collapse and artificial bone rejection reaction. The Maryland functionalevaluation showed that the foot function was excellent in 78 cases,good in 34 cases and fair in 8 cases. The satisfied rate was 93. 3%. Conclusion: The calcaneal fractures with joint compression treated with internal fixation combined primary artificial bone grafting can obtain favourable prognosis.
