乙肝患者HBV-DNA载量与外周血T细胞CD127及肝功能的相关性研究

目的通过检测乙型肝炎(乙肝)患者乙肝病毒脱氧核糖核酸(HBV-DNA)载量,探讨其与外周血T淋巴细胞表面CD127的表达水平及肝功能的相关性。方法将慢性乙肝病毒(HBV)感染患者64例按疾病类型分为慢性乙肝组43例和乙肝性肝硬化21例,另选择健康体检者20例作为对照组,采用流式细胞术检测乙肝患者及健康者外周血T细胞表面CD127的表达水平,实时荧光定量PCR法检测乙肝患者HBV-DNA并进行分析,同时采用全自动生化分析仪检测丙氨酸氨基转移酶(ALT)、总胆红素(TBIL)水平,并对各组检测值进行比较和相关性分析。结果乙肝组CD4+CD127+表达率与对照组比较差异无统计学意义(P>0.05)。乙肝组CD8+CD127+表达率、ALT和TBIL值及血清HBV-DNA定量显著高于对照组,且乙肝性肝硬化组ALT值显著低于慢性乙肝组,差异均有统计学意义(P<0.05和P<0.01)。HBV病毒载量的对数值与外周血T淋巴细胞表面CD127的表达呈负相关(r=-0.391,P<0.01),但HBV-DNA载量与肝功能无明显相关性。结论乙肝患者HBV-DNA与外周血T淋巴细胞表面CD127的表达水平明显相关,但与肝功能无明显相关性。

The correlation between serum HBV DNA level in Hepatitis B and the expression of CD127 and liver function

Objective To investigate the correlation between the progression of HBV infection and the expression of CD127 on the surface of T cells and liver function in HBV infected patients.Methods 64 patients with chronic Hepatitis B virus（HBV） infection were divided into chronic Hepatitis B group of 43 patients with Hepatitis B cirrhosis and 21 cases according to the disease types and the other 20 cases of healthy check-up were regarded as control group.HBV-DNA was quantitatively detected with realtime PCR from 64 patients with hepatitis B and 20 healthy controls and the expression of CD127 on the surface of T lymphocytes was detected by flow cytometry.Biochemistry analysator was used to detect ALT and TBIL.The indicators were compared and correlation was analyzed.Results There was no significant difference of CD4＋ CD127 ＋ expression rate between Hepatitis B group and control group（P0.05）.CD8＋ CD127＋ expression rate,ALT and TBIL value and serum HBV-DNA ration in Hepatitis B group were significantly higher than those in the control group,and ALT in liver cirrhosis group was significantly lower than that of chronic Hepatitis B group,and the differences were significant（P0.05 and P0.01）.HBV virus to the numerical and peripheral blood lymphocyte surface T the expression of CD127 showed negative correlation（r=0.391,P0.01）,but HBV-DNA load was not significantly associated with liver function..Conclusion HBV-DNA in Hepatitis B patients is obviously related to peripheral T lymphocyte surface CD127 expression level,but no significant correlation with liver function.
