支气管肺泡灌洗液中T-抗原检测对肺癌诊断的价值

目的 :探讨肺癌支气管肺泡灌洗液中T 抗原检测的诊断意义。方法 :收集 61例原发性肺癌和 3 1例非肺癌患者的支气管肺泡灌洗液 ,采用半乳糖氧化酶法检测痰液中T 抗原的敏感性和特异性。结果 :肺癌组T 抗原总阳性表达率为 68 3 % ,对照组BALF中T 抗原阳性率为 9 7%。中心型肺癌组支气管肺泡灌洗液中T 抗原阳性率 67 3 % ,纤支镜刷检细胞学阳性率为 61 2 % ,支气管肺泡灌洗液细胞学阳性率为 12 2 % ,支气管肺泡灌洗液中T 抗原的阳性率高于细胞学阳性检出率 ,二者比较差异有显著性 (P <0 .0 1)。周围型肺癌组支气管肺泡灌洗液中T 抗原阳性率为 71 4% ,纤支镜刷检细胞学阳性率为 5 0 0 % ,支气管肺泡灌洗液细胞学阳性率为 2 1 4% ,支气管肺泡灌洗液中T 抗原阳性率高于支气管肺泡灌洗液细胞学阳性率 ,二者比较差异有显著性 (P <0 .0 1)。结论 :支气管肺泡灌洗液中T 抗原有较高的敏感性和特异性 ,可作为肿瘤标志物为临床诊断提供重要线索

The study on diagnostic value of detection of T-antigen in bronchial alveolar lavage liquid in patient with primary lung cancer

Objective: To access the diagnostic value of detection of T antigen in bronchial alveolar lavage liquid (BALF) in patients with lung cancer.Methods: To collect BALF of patients with primary lung cancer and (61 cases) with non lung cancer (31 cases),to detect the T antigen with galactose oxidase.Results: The positive rates of T antigen in BALF of lung cancer group and non lung cancer are 68.3% and 9.7% respectively.In central primary lung cancer the positive rates of BALF T antigen,brush slide cytology and BALF cytology are 67.3%,61.2% and 12.2% respectively,the positive rate of T antigen is higher than BALF cytology( P<0.01) .In the peripheral lung cancer group the positive rates of BALF T antigen,brush slide cytology and BALF cytology are 71.4%,50.0% and 21.4% respectively,the positive rate of BALF T antigen is higher than BALF cytology ( P<0.01). Conclusion: The detection of T antigen as tumor marker in BALF is available to diagnose of lung cancer.
