2型糖尿病肥胖患者66例代谢紊乱临床分析

目的对比观察肥胖和非肥胖2型糖尿病(DM)的代谢状态,探讨2型DM肥胖患者代谢紊乱的特点。方法128例2型DM根据体重指数(BMI)分为肥胖和非肥胖2组,肥胖组66例,非肥胖组62例。检测两组血压、空腹血糖(FPG)、糖化血红蛋白(HbA1c)、空腹血浆胰岛素(Fins)、总胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白胆固醇(HDLC)、低密度脂蛋白胆固醇(LDLC)、血尿酸(BUA)、24h尿白蛋白排泄率(UAER),并进行比较。结果2型DM肥胖组收缩压(SBP)、BUA明显高于非肥胖组(P<0.05);HDLC肥胖组患者明显低于非肥胖组(P<0.05);2型DM肥胖组Fins、TG、LDLC明显高于非肥胖组(P<0.01);肥胖组舒张压(DBP)、TC、FPG、HbA1c较非肥胖组略高,但无统计学意义(P>0.05)。结论肥胖的2型DM患者有着严重的代谢紊乱,对肥胖的2型DM患者应进行综合治疗,改善其代谢紊乱,控制和减慢血管并发症的发展进程。

Clinical analysis of the metabolic disorder in 66 cases of type 2 diabetes mellitus with obesity

Objective To study the characteristics of the metabolic disorder of the obese patients with type 2 diabetes mellitus(DM) through analyzing their metabolic difference between the obese and non-obese patients.Methods On hundred and twenty-eight cases of type 2 DM were divided into 2 groups-obesity 66 cases and non-obesity 62 cases.The blood pressure(BP), fasting plasma glucose (FPG), HbA1c,fasting plasma insulin (Fins),total cholesterol (TC), triglyceride (TG),high dense protein cholesterol (HDL-C),low dense protein cholesterol l(LDL-C),blood uric acid (BUA) and urinary albumin excretion rate(UAER)were tested.Results The SBP,BUA in the obese group were higher than that in the non-obese group(P<0.05), HDL-C level significantly decreased in obesity group than that in non -obese group(P<0.05); Fins,TG,LDL-c in the obese group were higher than those in the non-obese group(P<0.01); while the DBP, TC,FPG and HbA1c in the obese group were higher than those in the non-obese group (P>0.05).Conclusion Metabolic disorder in obese patients with type 2 DM is even more serious. It is very important for obese patients with typ 2 DM to give combined treatment, improve the metabolic disorder, control and slow down the progress of vascular complication.
