呼吸机相关肺炎患者死亡的影响因素分析

目的探讨呼吸机相关肺炎（ventilator-associated pneumonia,VAP）患者死亡率的影响因素,为今后相关研究提供依据。方法应用SPSS 18.0软件,对进行机械通气的367例患者的临床资料,采用χ2检验和Logistic回归分析进行统计学分析。结果 VAP组死亡率高于非VAP组,两组差异有统计学意义（χ2=39.7,P〈0.01）。经多因素Logistic回归分析得出年龄、基础疾病、机械通气时间、口腔护理次数、呼吸机依赖、白蛋白〈30g/L是影响VAP死亡率的最主要因素。结论加强医护人员专业培训,根据患者个体差异制定个性化的治疗计划和相应的护理措施,从而降低呼吸机相关肺炎死亡率。

Analysis of influence factors of mortality for ventilator-associated pneumonia

Objective To explore the influence factors of mortality rate for VAP,provides the basis for future research.Methods The clinical data of 367 patients with mechanical ventilation were analyzed,using SPSS 18.0 software,conducted chi-square test and logistic regression to analysis.Results Mortality rate of VAP was higher than no VAP group,mortality rate was statistically significant differences（χ^2=39.7,P〈0.01）.After many factors Logistic regression analysis,the most important effect factor in mortality rate of VAP were age,basic diseases,duration of mechanical ventilation,oral nursing times,breathing machine dependent,albumen〈30g/L.Conclusion Strengthen medical personnel professional training and made the treatment plan of individual and corresponding nursing measures for individual patients,so as to reduce mortality rate of ventilator-associated pneumonia.
