我院呼吸科住院患者抗茵药物应用分析

目的探讨我院2010年呼吸科住院患者抗菌药物使用的合理性。方法采用回顾性分析方法，随机抽取呼吸科出院病历100份，对抗菌药物的用药方案、联合用药、用药起始时间、病原学检查、抗菌药物使用情况等进行统计并评价其合理性。结果100例呼吸科住院患者使用抗菌药物比例为99％，平均用药天数13d，其中二联用药比例占72％。用药频度前3位的药物分别为左氧氟沙星、阿莫西林克拉维酸钾和头孢唑肟；使用频率居前3位的抗菌药物依次为喹诺酮类、头孢菌素类、青霉素类；病原学送检率为77％。结论呼吸科住院患者的抗菌药物使用基本合理。但仍存在联合用药依据不足、时间依赖性抗菌药给药间隔不当等不合理用药情况，需要加强培训和监督管理。

Application analysis of antibiotic utility in respiratory department of our hospital

Objective To explore the rationality of antibiotic utility in the respiratory department of our hospital in 2010. Methods Restrospective analysis was used and 100 medical records were randomly selected from the respiratory de- partment. To evaluate the rationality of antibiotic regimens,combined drugs,starting time and use of antibiotics and pathogenic test was antibiotic utility. Results 100 cases of hospitalized patients in a respiratory department using antimicrobial drug use rate was 99%. There were 72% of patients having a combination of two antibiotics. The average duration of antibiotic treatment was 13 days.The top three antibiotics in the list of DDDs were levofloxaction, amoxillin and clavulanate potassium and cefti- zoxime.The top three most frequently used antibiotics were levofloxaction,amoxillin and clavulanate potassium and ceftizoxime. Frequency of pathogenic examination was 77%. Conclusion Antibiotic use in our respiratory department is reasonable, but there are some problems, such as insufficient evidence for combined antibiotic therapy, inappropriate dosing interval for time- dependent antibiotics, which require strengthened training and supervision.
