宫腔镜在绝经期取器困难的应用

目的提高绝经期取器困难的手术成功率 ,减少并发症。方法对56例常规取环失败的妇女 ,应用宫腔镜技术诊断定位后 ,用钩取或钳取法取出。结果56例在宫腔镜下观察到宫颈缩窄18例 ,宫颈粘连2例 ,环位正常20例 ,嵌顿17例 ,单纯断裂7例 ,断裂 嵌顿6例 ,残留6例 ,均一次性成功取出。结论宫腔镜能在直视下诊断节育环的位置、形态 ,联合B超诊断可明确嵌顿环的深度 ,从而指导手术操作 ,对于钩取、钳取困难者可在宫腔镜直视下经操作孔用异物钳取出 ,所以宫腔镜在绝经期困难取器中有其独特的优越性 ,一旦发生取器困难应在宫腔镜诊断后取器

Application of hysteroscope in difficult removal of intrauterine device on climateric

Objective To improve the rate of success and decrease the complication in the difficult removal of IUD on climacteric. Methods 50 cases women failure in removal of IUD as usual were extracted IUD by hook or forceps holder after level diagnosis by hysterscope. Results 56 cases were extracted successful IUD by once. There were cervical spasm 18 cases, cervical adhesions 2 cases, normal IUD 20 cases, incarcer ation 17 cases, fragmentation 7 cases, incarceration and fragmentation 6 cases, surrival 6 cases. Conclusions The states of IUD by the hysterscope and the depth of IUD can be diagnosed when incarcerated combined with the use of ultrasonic wave. IUD can be extracted by hysterscope when its difficult. So that it is very superior to use the hysterscope when IUD can't be extracted on climacteric.
