| 摘 要： | 采用小血管吻合技术,行游离空肠移植,喉咽及颈段食管重建10例,9例成活,术后伤口愈合顺利,并发症少,吞咽功能恢复良好。本手术有两项突破性改进,一是将喉的未受累部分保留下来,经过整复重建了喉功能;二是移植肠段裂开移植,这适于喉部分保留或全喉保留及咽部分保留病例,扩大了本手术的适应证,有利于喉功能的恢复。本手术一期完成,成功率高、安全可靠,值得推广。

| Abstract: | Reports 10 cases of transplantation of free jejunal segment to reconstruct hypopharynx and cervical esophagus.Eight cases were after an extensive resection of pharyngeal carcinoma,one was for reconstruction of the pharyngo- esophageal defect due to cervical trauma in an accident,one was for corrosive atresia of hypopharynx and cervical.Nine were survived.Among them,the laryngeal function were completely restored in 4,and partially restored in 3. One was failure due to long time ischemia of segment because of microscope trouble. Surgical procedures were improved as follows:1.the unaffected larynx was preserved either totally or partially to restore the fullor partial laryngeal function, 2.jejunal segment was split partially or totally longitudinally along the opposite side of mesentery for reconstructing pharyngoesophageal irregular defect. |
