三位一体模式对社区2型糖尿病的监控

目的探讨三位一体模式对社区2型糖尿病的监控效果。方法选取本辖区（塘头社区）150例2型糖尿病患者为观察组，以三位一体模式为患者及家庭进行综合干预，同时选取另外150例患者为对照组，按一般的模式进行干预。对比两组患者干预前后空腹血糖、餐后2h血糖、糖化血红蛋白变化情况、患者对糖尿病的认知率、治疗率及控制率。结果干预后，观察组患者空腹血糖、餐后2h血糖、糖化血红蛋白都明显下降，与干预前比较差异有统计学意义（P〈0．05）．与干预后对照组相比差异有统计学意义（P〈0．05）。观察组患者认知率、治疗率及控制率．与对照组相比差异有高度统计学意义（P〈0．01）。结论三位一体模式能有效地控制社区2型糖尿病患者病情，是一个值得推广的全新模式。

Monitoring 2 type glycuresis of community by the trinity mode

Objective To discuss efficacy of monitoring 2 type glycuresis of community by the trinity mode. Methods 150 cases of patients with 2 type diabetes of the community were chosen as observation group, patients and family were comprehensive intervened by the trinity mode at the same time, another 150 patients were selected as control group, which had intervened by the general mode. Compared with two groups of fasting blood glucose, postprandial two-hour blood glucose, glycosylated hemoglobin of before and after the intervention, and compared with two groups of recognition rate of diabetes, therapy rate and control rate. Results After intervened, the fasting blood glucose, postpran- dial two-hour blood glucose, glycosylated hemoglobin of patients were obvious dropt in observation group. Compared with before intervention were obvious difference （P 〈 0.05）;compared with the control group of after intervention were obvious difference （P 〈 0.05）; recognition rate, therapy rate and control rate in observation group compared with the control group were obvious difference （P 〈 0.01）. Conclusion The trinity mode can effectively controlling the status of 2 type glycuresis of community. It is worthy of popularizing.
