Corin Rotaglide全膝关节假体置换26例

回顾性分析2007-05/2008-05河南省洛阳正骨医院膝部损伤科收治的骨性关节炎患者15例及类风湿性关节炎患者11例(共34膝)，男7例，女19例，年龄52~67岁；均为初次行膝关节置换，所用假体为英国Corin Rotaglide全膝关节假体。记录置换时间，置换后住院天数，参照膝关节协会评分对比置换前后膝关节功能，观察置换中并发症并摄标准膝关节正侧位X射线平片观察假体位置。26例患者均获随访，平均11个月。膝关节活动度从置换前平均84°(40°~95°)增加到置换后平均104°(80°~120°)，膝关节评分从置换前平均39分(10~67分)，提高到随访时平均87分(10~100分)，功能评分从置换前平均38分(0~60分)提高到随访时平均76分(20~100分)。31膝获得肢体的良好对线，股骨及胫骨假体位置良好。1例胫骨假体后倾不足，2例胫骨假体残留2°~3°内翻，1例胫骨假体与骨组织界面存在非进展性透亮带。置换过程中发生副损伤或并发症4例，其中内侧副韧带股骨起点损伤1例，行可吸收螺钉固定并加用可吸收线、丝线缝合，置换后带支具锻炼，恢复良好；内翻畸形未完全矫正2例，至今未出现不适；脑梗死1例，经内科治疗后未遗留神经症状。提示Corin Rotaglide全膝关节置换减少了假体磨损和潜在的松动，尤其适合年轻活动量大的患者。

Total knee replacement in 26 cases using Corin Rotaglide system

From May 2007 to May 2008, 15 cases of osteoarthritis and 11 cases of rheumatoid arthritis, a total of 34 knees involved, including 7 males and 19 females, aged 52-67 years, were admitted to Depratment of Knee Joint, Luoyang Hospital of Orthopaedics and Traumatics. All patients underwent primary total knee replacements using Corin rotaglide knee replacement system. Replacement duration and time of hospital days after surgery were recorded, and Knee Society Scores were used to evaluate knee function. Complications were observed, and X-ray was performed to observe prosthesis location. All cases were followed up for average 11 months. The mean range of motion was improved from preoperative 84°(40°-95°) to postoperative 104°(80°-120°); the mean preoperative Knee Society Scores were increased from preoperative 39 scores (10-67 scores) to 87(10-100), and the function was improved from 38 scores (0-60 scores) to postoperative 76(20-100). Alignment was obtained in 31 knees, with well localized femoral and tibial prostheses. The tibial prosthesis of 1 case developed inadequate retroversion; of 2 cases remained 2°-3° inversion; non-progressive radiolucent zone was found between tibial prosthesis and bone interface of 1 case. In addition, 4 cases developed complications, including 1 case of medial collateral ligament femoral starting point injury, which was cured by screw fixation and absorbable line suture; 2 cases of incomplete rectification of inversion; and 1 case of cerebral infarction. The femoral prosthesis of Corin rotaglide knee replacement system can reduce wearing of prosthesis and loosening. It fits well the young people.
