白内障术后视网膜脱落发生率及影响因素分析

目的探讨白内障术后视网膜脱落发生率及影响因素。方法回顾性分析2016-01至2018-12期间在哈尔滨市眼科医院治疗的4120例白内障患者的临床资料,根据患者术后1年内是否发生视网膜脱落分为脱落组(n=39,39只眼)与未脱落组(n=4081,4081只眼)。采用多因素Logistic回归模型分析白内障术后视网膜脱落的影响因素。结果多因素Logistic回归分析结果显示,年龄≥60岁、存在高度近视、存在眼外伤、人工晶体植入睫状沟、后发性白内障、后囊膜破裂及糖尿病性视网膜病变是影响白内障术后视网膜脱落的危险因素。结论本研究术后视网膜脱落率为0.95%,高龄、手术原因与存在并发症或原发症患者,术后可能会出现视网膜脱落,出院时应告知其视网膜的临床症状,并定期体检,以便发现术后视网膜脱落情况并进行及时治疗,降低术后视网膜脱落发生率。

The incidence and influencing factors of retinal detachment after cataract surgery

Objective To investigate the incidence and influencing factors of retinal detachment after cataract surgery.Methods The data of 4,120 cases of cataract treated in Harbin Eye Hospital from January 2016 to December 2018 were analyzed retrospectively.According to whether there was retinal detachment within one year after surgery,the patients were divided into detachment group(n=39,39 eyes)and non-detachment group(n=4,081,4,081 eyes).Multivariate Logistic regression model was used to analyze the influencing factors of retinal detachment after cataract surgery.Results In 4,120 cases of this study,39 cases of post-operative retinal detachment occurred;the results of multivariate Logistic regression analysis showed that age≥60 years old,high myopia,eye injury,IOL implantation into ciliary sulcus,posterior cataract,posterior capsule rupture and diabetic retinopathy were the risk factors of postoperative retinal detachment.Conclusions The rate of post-operative retinal detachment is 0.95%.The elderly,causes of operation and complications or primary diseases may lead to retinal detachment.Patients should be informed of the clinical symptoms of the retina when discharged from the hospital,and regular physical examination should be carried out in order to find out the situation of post-operative retinal detachment and carry out timely treatment,so as to reduce the rate of post-operative retinal detachment.
