外伤性脑出血患者术后感染病原菌分布及预后研究

摘    要:目的分析外伤性脑出血患者术后感染病原菌分布,探究感染对患者预后的影响。方法选取2016年5月至2018年2月105例外伤性脑出血术后出现感染患者为感染组,同期外伤性脑出血术后无感染患者82例为未感染组,同期入院行健康检查的正常者70例为对照组。观察感染组病原菌分布,并比较三组MMSE评分及血清NGF、MDA、OSM水平。结果感染组105例患者,共检出117株病原菌,菌种以革兰阴性菌较多,占52.99%。感染组MMSE评分、OSM、NGF水平低于未感染组和对照组,MDA水平高于未感染组和对照组(P<0.05);未感染组MMSE评分、OSM、NGF水平低于对照组,MDA水平高于对照组(P<0.05)。结论外伤性脑出血术后感染者致病菌以革兰阴性菌多见,感染者MMSE评分、OSM及NGF水平降低,MDA水平上升。

Abstract:Objective To analyze the distribution of pathogenic bacteria in patients with traumatic cerebral hemorrhage complicated by postoperative infection, and to explore the influence of infection on patients’ prognosis. Methods From May2016 to February 2018, 105 patients with traumatic cerebral hemorrhage complicated by postoperative infection were selected as infection group, 82 cases of traumatic cerebral hemorrhage without postoperative infection at the same time were selected as uninfected group, and 70 normal patients admitted in hospital for health examination at the same time were selected as control group. The pathogenic bacteria distribution of the infected group was observed, and the MMSE score and levels of serum NGF, MDA and OSM were compared among the three groups. Results A total of 117 strains of pathogenic bacteria were detected in 105 patients of the infection group, among which Gram-negative bacteria accounted for 52.99%.The MMSE score, OSM and NGF levels of the infected group were lower than those of the uninfected group and the control group, and the MDA level was higher than that of the uninfected group and the control group(P<0.05);the MMSE score,OSM and NGF levels of the uninfected group were lower than those of the control group, and the MDA level was higher than that of the control group(P<0.05). Conclusion The Gram-negative bacteria are dominated in patients with traumatic cerebral hemorrhage complicated by postoperative infection, bringing about the lower MMSE score, OSM and NGF levels and higher MDA level.
