局部麻醉下骨髓采集术的安全性观察

目的　为恶性血液病及实体瘤大剂量化疗联合造血干细胞移植探讨一种方便、有效、安全的采集骨髓方法。方法　2 3例采集术于采集前3 0min肌注杜冷丁5 0mg、安定10mg ,半小时后,用0 .1%利多卡因2 0 0～40 0mg多部位局麻,采集骨髓2 0 0～80 0mL。采集过程中回输异体压积红细胞或自体血,同时观察疼痛情况、监测生命体征。结果　全部病例生命体征稳定。术中疼痛的VAS评分(视觉模拟评分法) 1分2人(8% ) ,2分19人(84% ) ,3分2人(4 % )。术后第2天患者即可自由活动,无麻醉相关不良反应。结论　局麻下采集骨髓简便、安全,值得推广使用。

To Observe the Safety of Bone Marrow Collection by Local Anesthesia

Objective To explore a safe convenient and effective method to collect bone marrow.Method: 50 mg pethidine and 10mg diazepam were injected into 23 cases including 6 patients and 17 normal donors before collection, then 200-800 mL bone marrow was collected by supporting multiple local anesthesia with 0.1% 200-400 mg Lidocaine. During harvesting, homologous or autologous blood were transfused to them. Vital sign and the ache were monitored.Results Vital sign of 23 cases remained steady. The VAS grade about their ache during harvesting was 1,2 cases (8%); 2,19 cases (84%); 3,2 cases (8%), and no side-effect occurred associated with anesthesia.Conclusion Colleting bone marrow in local anesthesia is a safe and simple method, which is worth spreading and applying.
