慢性硬膜下血肿钻孔冲洗引流术中两种冲洗液疗效对比

目的对比钻孔引流术治疗慢性硬膜下血肿患者人工脑脊液和等渗0.9%氯化钠注射液2种冲洗液(均为37℃)的疗效差异。方法从2008年4月至2010年1月,收治慢性硬膜下血肿患者(均为首次发病)80例,随机分为人工脑脊液或0.9%氯化钠注射液冲洗2组,前瞻性分析钻孔冲洗引流术中使用不同冲洗液的术后疗效。结果 80例患者均采用钻孔冲洗引流术;术中人工脑脊液冲洗组40例,治愈38例,复发2例,无死亡;术中0.9%氯化钠注射液冲洗组40例,治愈34例,复发6例,无死亡;2组间治愈率、复发率差异无统计学意义。结论钻孔冲洗引流术是治疗慢性硬膜下血肿的有效方法,与等渗0.9%氯化钠注射液相比,人工脑脊液冲洗并不能明显降低血肿的复发率。

Comparison of two kinds of introperative irrigation solutions in therapy of chronic subdural hematoma

Objective To adopt burr hole evacuation and drainage to treat chronic subdural hematoma(CSDH),comparing two kinds of introperative irrigation solutions′(warm isotonic saline and artificial CSF) therapeutic effects.Methods Eighty CSDH patients were randomly divided into 2 groups(performed with warm isotonic saline or artificial CSF irrigating respectively),received burr hole evacuation and drainage and were followed up for long-term effect.Results In artificial CSF irrigating group,38 of 40 cases were cured,2 cases relapsed,no death;in isotonic saline irrigating group,34 in 40 cases were cured,6 cases relapsed,no death.Conclusion Drilling drainage is an effective option to treat CSDH,and the use of intraoperative irrigation with warm artificial CSF cannot remarkably reduce the recurrence.
