乌司他丁对急性百草枯中毒致肺损伤的临床观察

目的：观察乌司他丁对急性百草枯（PQ）中毒致肺损伤临床疗效。方法：将41例急性PQ中毒患者随机分为治疗组21例和对照组20例，所有患者均予洗胃、血液灌流＋血液透析（HP＋HD）及甲基强的松龙静脉注射等综合治疗，治疗组在对照组基础上加用乌司他丁20万单位＋O．9％生理盐水100ml静脉滴注，每日2次，连续3-5d。比较两组动脉血气、X线胸片（第1、3、5天）变化、多脏器功能障碍综合征（MODS）（≥3个脏器）发生率、病死率、死亡病例的存活时间。结果：治疗后两组的动脉血气及X线胸片等较治疗前均有显著改善（P〈0．01），且治疗组明显优于对照组（P〈0．05）；两组MODS发生率、病死率与死亡病例存活时间无明显差异（P〉0．05）。结论：乌司他丁可改善PQ中毒后肺损伤患者动脉血气及x线胸片，对急性PQ中毒肺损伤有积极治疗作用。

Curative Effects of Ulinastatin on Acute Lung Injury after Paraquat Poisoning

Objective：To observe the curative effects of Ulinastatin on acute lung injury after paraquat （PQ） poisoning. Methods： 41 patients with acute PQ poisoning were randomly divided into treatment group （n=21） and control group （n=20）. The control group was accepted sequential blood purification （hemoperfusion and hemodialysis） combined with comprehensive such as gastric lavage, intravenously infused methylprednisolone, and the treatment group was treated by Ulinastatin 2x106 units for 3-5 consecutive days based on the control group. The artery blood gas, X-ray film changes, the occurrence rate of multiple organ dysfunction syndrome （MODS）, mortality and survival time of the death cases were compared between the two groups. Results： Compared with before treatment, the artery blood gas and X-ray film changes were significantly better in the two groups （P〈0.01） ,especially in the treatment group compared with the control group （P〈0.05）. The occurrence rate of MODS,mortality and survival time of the death cases were not significantly different between the two groups （P〉0.05）. Conclusion： Ulinastatin can significantly improve the artery blood gas and X-ray,which has a positive therapeutic effect on pulmonary damage in acute PQ poisoning.
