连续性血液净化治疗重症急性有机磷中毒的临床观察

目的：探讨连续性血液净化（CBP）治疗重症急性有机磷农药中毒（ASOPP）的临床效果。方法：将2009年7月至2012年12月我科收治的ASOPP患者56例随机分为治疗组及对照组各28例，所有患者常规给予洗胃、导泻、补液、使用阿托品和胆碱酯酶复能剂、气管插管、呼吸机辅助呼吸等综合治疗，入ICU后即给予血液灌流1次，治疗组连续3天CBP治疗，比较两组在ICU天数、胆碱酯酶活力恢复时问、昏迷时间及死亡率。结果：与对照组比较．治疗组在ICU天数缩短、胆碱酯酶活力恢复快、昏迷时间缩短和死亡率降低，差异有统计学意义（尸〈0．05）。结论：对ASOPP患者，在常规救治基础上辅以CBP治疗能持续清除体内毒物，缩短病程，降低患者死亡率。

Clinical Observation of Patients with Acute Severe Organophosphorus Pesticide Poisoning Treated by Continuous Blood Purification

Objective：To explore the clinical effects of patients with acute severe organophosphorus pesticide poisoning （ASOPP） treated by continuous blood purification （CBP）. Methods： 56 patients with ASOPP were randomly divided into treatment group and control group from Jul 2009 to Dec 2012, and each group was 28 cases. All the cases were given conventional gastric lavage, catharsis, rehydration, atropine and cholinesterase reactivator, tracheal intubation, mechanical ventilation therapy, blood perfusion 1 time after into the ICU, 3 consecutive days of CBP in the treatment group addedly. The ICU hospitalized days, cholinesterase recovery time, the duration of coma and death were compared between the two groups. Results： Compared with the control group, the ICU hospitalized days was shorter, cholinesterase recovery was faster, the coma time was shorter and mortality was lower in the treatment group, and the difference was statistically significant, all P 〈 0.05. Conculsions： For the patients with ASOPP, based on routine treatment with CBP therapy can continue to remove toxic, shorten the course, reduce the mortality.
