新生儿呼吸窘迫综合征的临床X线诊断

目的 探讨新生儿呼吸窘迫综合征(IRDS)的X线表现及其动态变化。方法 回顾近三年来62例经临味诊断、对症治疗并且有完整X线资料的新生儿呼吸窘迫综合征病例结果 62例中X线表现与临床相符合55例(88.71％),不典型表现7例(11．29％)；其中合并肺炎12例，先天性心脏病2例(PDA)，肺出血3例，气胸5例。根据X线征象分期，轻度37例，中度11例，重度7例。结论 X线平片是协助临床诊断新生儿呼吸窘迫综合征的重要方法之，应结合临床提高X线诊断能力。

The clinical radioiogic diagnosis of idiopathic respiratory distress syndrome in newborn

Objective To investigate the X -ray findings an d dynamic changes of the idiopathic r espiratory distress syndrome (IRDS)in the newborn.Methods The clinical manifestations and chest X -ray findings of 62newborn with I RDS collected from January 1999to November 2002were analyzed retrosp ectively.Results The study suggested that the X -ray fi ndings corresponding to the clinica l manifesta-tions were found in 55cases(88.71%)while the both to be noncorrespondin g were found in 7cases(11.29%).Among these newborn with IRDS,there were 12cases complicate d with pneumonia,2congenital heart disease,3pneumorrhagia,and 5pneumothorax.According to the severity of X -ray findings,55cases with X -ray findings corresponding t o clinical manifestations were divi ded into degrees:mild(n =37),moderate(n =11)and severe(n =7).Conclusion chest X -ray film is very helpful to di agnose IRDS in newborn especially th e integration of X -ray and clinical examinations c an significantly improve the diagnostic accuracy of IRDS.
