食管心房调搏对隐匿性房室旁道定位诊断指标的研究

摘    要:目的:探讨食管心房调搏术对隐匿性房室旁道定位诊断的指标。方法:回顾分析射频消融成功145例隐匿性房室旁道患者房室折返性心动过速时的食道心电图逆行P波(P-ESO)与V1导联逆行P波(P-V1),测量RP-ESO和RP-V1间期,根据RP-ESO、RP-V1和RP-ESO-RP-V1测量结果,将食管心房调搏术诊断隐匿性房室旁道定位结果分别与射频消融结果对比分析。结果:食管心房调搏术以指标一对左侧隐匿性房室旁道的敏感性、准确性与指标二相比有显著性差异(P<0.05),特异性无显著性差异(P>0.05);对右侧隐匿性房室旁道的敏感性两者有显著性差异(P<0.05),特异性和准确性无显著性差异;指标一诊断间隔旁道的特异性和准确性均较高。结论:食管心房调搏术以指标一对隐匿性房室旁道定位的诊断有重要价值。

Study concering the diagnosis of concealed accessory atrioventrieular pathway by transesophaged atrical pacing
Abstract:Objective: To investigate the index of the location diagnosis of concealed accessory atrioventricular pathway by transesophaged atrical pacing(TEAP).Method: We retrospectively analysed 145 cases of concealed accessory atrioventricular pathway which were treated successfully by RFCA when AVRT happened. We noted the retrograde P- wave interval in TEAP (P~-_ ESO) and V1 lead(P~-_ V1), measured the R P~-_ ESO and R P~-_ V1 interval , adopted two indexes to diagnose the concealed accessory atrioventricular pathway, and compaired the location result with RFCA . Result: The sensitivity and accuracy of location diagnosis of left side accessory atrioventricular pathway by No.1 index were significantly different compared with No.2 index, There was no significant difference about the specificities; The sensitivity of location diagnosis of right side concealed accessory pathway by no.1 index had significant difference compared with no.2 index, No significant difference appeared on sensitivity and accuracy ; About septal location accessory pathway , index one had higher specificities and accuracy.Conclusion: The No.1 index had important value to localize the concealed accessory atrioventricular pathway by TEAP.
