髋臼骨折合并骶髂关节脱位的手术治疗

【目的】探讨髋臼骨折合并骶髂关节脱位的手术方法及疗效。【方法】对 2 3例髋臼骨折合并骶髂关节脱位患者采用早期手术复位内固定及术后早期功能锻炼治疗 ,并对术后X线片复查情况及髋关节功能恢复情况进行分析。【结果】2 3例均获得随访 ,随访时间 0 .5～ 7.5年 ,平均 3.5年。术后X线片复查示骶髂关节脱位及髋臼骨折复位满意 ,骶髂关节均获融合 ,关节功能优 14例 ,良 5例 ,可 3例 ,差 1例 ,优良率82 .6 %。无一例发生血管神经损伤等严重并发症。【结论】对髋臼骨折合并骶髂关节脱位患者采用早期手术复位内固定治疗可获得满意效果。

Operative Treatment of Acetabular Fracture Complicated with Iliosacral Dislocation

ObjectiveTo study the operative method and efficacy for the fracture of acetabulum and dislocation of iliosacral joint.Twenty-three cases of fracture of acetabulum with dislocation of sacroiliac joint were treated by operative reduction ,internal fixation and functional exercise at earlier period.The result of functional restoration was analysed.After follow-up from 0.5 to 7.5 years, average 3.5 years,the reduction was satisfactory and sacroiliac joints fused.Joint functions were excellent in 14 cases, good in 5 cases, fair in 3 cases, poor 1 case.The rate of excellent and good function was 82.6%.Conclusion]The fracture of acetabulum with dislocation of sacroiliac joint need operative reduction , internal fixation and functional exercise at earlier period. Anatomic reduction can get good efficacy about functional recovery.
