同步调强放化疗治疗局部晚期鼻咽癌40例

[目的]观察同步调强放化疗治疗局部晚期鼻咽癌的近期疗效及毒副反应。[方法]2006年4月至2008年4月,80例局部晚期鼻咽癌随机分为同步放化疗组（同步组,n=40）和单纯放疗组（单放组,n=40）。同步组接受调强放疗加顺铂（80mg/m2）治疗,单放组接受单纯调强放疗。[结果]同步组和单放组近期疗效比较无显著性差异（P〉0.05）。2年局部控制率同步组为97.5%（39/40）,单放组为92.5%（37/40）（P〉0.05）。2年无远处转移生存率同步组为95.0%（38/40）,单放组为80.0%（32/40）（P〈0.05）。同步组Ⅲ/Ⅳ级白细胞下降及恶心、呕吐的发生率明显高于单放组（P〈0.05）。[结论]同步调强放化疗对局部晚期鼻咽癌疗效较好,且毒副反应可耐受。

Concurrent Intensity Modulated Radiochemotherapy in the Treatment for 40 Cases with Locally Advanced Nasopharyngeal Carcinoma

[Purpose] To investigate the short-term effect and toxicity of concurrent intensity modulated radiotherapy（IMRT） and chemotherapy in the treatment for locally advanced nasopharyngeal carcinoma.[Methods] From April 2006 to April 2008,80 cases with locally advanced nasopharyngeal carcinoma were randomly divided into concurrent chemoradiotherapy group（CCRT,n=40） and radiotherapy alone group（RT,n=40）.Patients in CCRT group were received IMRT combined chemotherapy with DDP（80mg/m2）,and patients in RT group were received IMRT alone.[Results] The short-term response in CCRT group and in RT group was similar（P0.05）.The 2-year local control rate in CCRT group and in RT group was 97.5%（39/40） and 92.5%（37/40） respectively（P0.05）.The 2-year distant metastasis-free survival rate in CCRT group and in RT group was 95.0%（38/40） and 80.0%（32/40）,respectively（P0.05）.The incidence of grade Ⅲ and Ⅳ leucopenia and nausea and vomiting in CCRT group was significantly higher than that in RT group（P 0.05）.[Conclusion] Concurrent IMRT and chemotherapy for locally advanced nasopharyngeal carcinoma is effective and tolerable.
