音乐放松疗法对肝癌动脉化疗栓塞术病人生命体征与焦虑状态的影响

目的]探讨音乐疗法对肝癌经导管动脉化疗栓塞术病人的心率、呼吸、血压及焦虑程度的影响。方法]将102例行经导管动脉化疗栓塞术的肝癌病人随机分为干预组与对照组,对照组接受肝癌介入常规护理,干预组在常规护理的基础上进行音乐疗法。观察两组术前、术中、术后的心率、呼吸、血压变化,并采用状态焦虑量表对手术前后两组病人的焦虑程度进行评估。结果]术前对照组与干预组焦虑评分比较差异无统计学意义（P〉0.05）,术后干预组焦虑评分明显低于对照组,差异有统计学意义（P〈0.001）,组内比较对照组手术前后焦虑评分无明显变化（P〉0.05）,干预组术后焦虑评分较术前明显降低,差异有统计学意义（P〈0.001）。两组术前心率、呼吸、血压比较差异无统计学意义（P〉0.05）,术中对照组心率、血压变化幅度大,表现为心率加快,血压升高的趋势,而干预组心率、血压较稳定,两组心率、血压比较差异有统计学意义（P〈0.05或P〈0.001）。结论]应用音乐疗法可明显改善肝癌动脉化疗栓塞术病人的应激状态,稳定术中生理指标、减轻焦虑程度。

Influence of music relaxation therapy on vital signs and anxiety of liver cancer patients accepting transcatheter hepatic arterial chemoembolization

Objective：To probe into the influence of music therapy on physiological indexes（heart rate,respiration and blood pressure） and the anxiety of liver cancer patients accepting transcatheter hepatic arterial chemoembolization.Methods：A total of 102 patients accepting transcatheter hepatic aterial chemoembolization were randomly divided into intervention group and control group.Patients of control group received routine care.Patients of intervention group received music therapy based on the routine care.Then changes of heart rate,respiration and blood pressure of all cases were observed before,during,and after the operation respectively.And anxiety degrees of all patients were evaluated by adopting the state anxiety inventory（S-AI） before and after the operation.Results：There was no statistical significant difference between the two groups in terms of anxiety scores before the operation（P〉0.05）.However,the anxiety score of intervention group was significantly lower than that of control group after the operation（P〈0.001）.In terms of the anxiety level of control group cases,there was no statistical significant difference between before and after the operation（P〉0.05）.However,in terms of the anxiety score of intervention group cases after the operation was significantly lower than that of before the operation（P0.001）.In terms of heart rate,respiration and blood pressure,there were no statistical significant differences between the two groups before the operation（P〉0.05）.During the operation,the heart rate and blood pressure of control group cases obviously changed.They manifested as fast heart rate and higher tendency of blood pressure.However,both the heart rate and blood pressure of patients in intervention group were stable.In terms of the changes of heart rate and blood pressure,there were statistical significant differences between the two group cases（P〈0.05 or P〈0.001）.Conclusion：Music therapy can improve stress state of liver cancer patients accepting transcatheter hepatic arterial chemoembolization.It can stabilize intraoperative physiological indexes of patients and reduce their anxiety degree as well.
