胰岛素抵抗对高血压患者左室肥厚及早期左心功能的影响

目的探讨胰岛素抵抗（IR）对高血压患者左室肥厚与早期左心功能的影响。方法选取高血压患者60例（A组）,其中无左室肥厚者30例（A1组）,左室肥厚组30例（A2组）,另选取健康对照组30例（B组）。常规超声心动图检查,并计算左室重量指数（LVMI）、左室Tei指数,测定空腹血糖及空腹胰岛素水平,计算胰岛素抵抗指数（HomaIR）。结果 A组与B组相比,Tei指数及Homa IR增加（P〈0.05）,LVMI高于对照组（P〈0.05）;A2组与A1组比较,Tei指数及Homa IR增加（P〈0.05）,LVMI明显高于对照组（P〈0.01）。结论 IR与高血压患者左室肥厚的形成密切相关,Tei指数可用于早期评估高血压患者左室功能。

Preliminary study on effect of insulin resistance on left ventricular hypertrophy and left ventricular function in patients with hypertensive on the early stage

Objective To evaluate the effect of insulin resistance on left ventricular hypertrophy and left ventricular function in patients with hypertensive on the early stage.Methods Transthoracic echocardiography was performed in 30 normal subjects(group B) and in 60 patients hospitalized with hypertensive(group A),in which 30 patients with Left ventricular hypertrophy(group A1),and 30 with normal Left ventricular wall Thickness(group A2).The Tei index and left ventricular mass index indices(LVMI)were measured.IR was obtained using the Insulin resistance index(Homa IR) according to the appropriate formula.Results Tei index,LVMI and Homa IR were increased in group A compared to group B(P<0.05).Tei index and Homa IR were increased group A2 compared to group A1(P<0.05).LVMI was significantly increased in group A2 compared to group B(P<0.01).Conclusion In patients with hypertensive,IR is very common.Tei index can react the general heart function sensitively and has a good correlation between Homa IR,so it is a appropriate index to evaluate the early heart function of hypertensive patients with IR.
