胆囊动力学变化对胆囊结石形成影响的实验观察

摘    要:目的 探讨生理条件下胆囊动力学变化与胆囊结石形成之间的关系。方法 37只家兔随机分为3组:(1)对照组13只,饲以标准饲料;(2)成石组14只,饲料中加入1.2%胆固醇;(3)防石组10只,在与成石组相同条件下.加入能促进胆囊收缩的红霉素。4周后各组行放射性核素肝胆显像、胆囊动力学分析,检测结石形成率,测定胆汁中胆固醇浓度。结果 第1~3组生成结石者分别为0只、12只和0只,第2组与第3组胆汁中胆固醇浓度显著高于第1组,而第2组各时相排胆分数及排胆率明显低于第1组和第3组。结论 胆囊排空障碍是结石形成的重要因素.核医学肝胆显像及胆囊动力学分析在预测结石形成方面有重要的临床实用价值。

Abstract:Objective To explore the relationship between kinetics of gallbladder and gallstone formation. Methods Thirty seven rabbits were randomized into three groups: (1) normal control with standard feed( n =13); (2) 1.2% cholesterol was added into the feed(n=14); (3) 1.2% cholesterol and erythromycin( n =10). All animals were fed for four weeks before measurement. Results There was no gallstone formation in group 1 and group 3,but there were 12 cases in group 2. The level of cholesterol in bile in group 2 and group 3 was higher than that of group 1, but the GBEF and gallbladder emptying rate in group 2 were lower than those of group 1 and 3. Conclusions Gallbadder emptying plays an important role in gallstone formation. Hepatobiliary imaging and gallbladder dynamics analysis have much clinical significance for prognosing gallstone formation.
