老年高血压合并阻塞性睡眠呼吸障碍患者血压昼夜节律变化

目的探讨阻塞性睡眠呼吸障碍对老年高血压患者血压昼夜变化的影响。方法人选老年高血压患者106例，根据多导睡眠仪监测分为单纯高血压组54例、高血压合并轻度阻塞性睡眠呼吸暂停综合征组(高血压加轻度OSAS组)25例和高血压合并中、重度阻塞性睡眠呼吸暂停综合征组27例。行24小时动态血压监测．比较睡眠呼吸参数对血压昼夜节律的影响。结果老年高血压合并轻度和中、重度阻塞性睡眠呼吸暂停综合征组昼夜血压曲线呈非杓型的分别占52．0％和74．1％。与单纯高血压组比较差异有统计学意义。结论应重视对老年非杓型血压患者睡眠障碍的监测。

Clinical study on circadian blood pressure variability in elderly hypertensive patients with obstructive sleep apnea syndrome

Objectives To explore the influence of obstructive sleep apnea syndrome (OSAS)on circadian blood pressure variability in elderly hypertensive patients. Methods One hundred and six elderly hypertensive patients were measured by polysomnography (PSG). They were divided into three groups bases on PSG: hypertensive without OSAS group, hypertensive with mild OSAS and hypertensive with moderate-severe OSAS. All patients used 24-hour ambulatory blood pressure monitoring (ABPM). The data from PSG and ABPM were campared. Results In the hypertensive with mild OSAS and hypertensive with moderate-severe OSAS group, 50.0% and 74.1% patients presented non-dipper profile. It was higher than hypertensive without OSAS group. Circadian blood pressure variability is correlated to the severity of OSAS. Conclusions Circadian blood pressure variability is significantly related to OSAS severity. The elderly non-dipper patients should be sujested to monitor by PSG.
