女性不孕患者血清CA125测定的临床价值

为了解女性不孕患者血清CA125含量与子宫内膜之间的临床关系。应用放射免疫法测定62例不孕妇女血清CA125含量,结合腹腔镜检查,诊断子宫内膜异位症。结果内膜异位症组月经期血清CA125含量为83.2±82.8U/ml,高于盆腔广泛性炎变粘连组月经期32.4±19.5U/ml(P>0.01);高于单纯性盆腔炎组月经期29.6±12.1U/ml(P<0.01);仅用月经期CA125含量诊断子宫内膜异位症受盆腔炎症干扰,阳性预测值低。血清CA125值(月经期/卵泡期)>2.0灵敏度83.3％,特异性84.2％。建议用血清CA125比值>2.0早期筛选子宫内膜异位症。

Clinical Evaluation of Serum Ca125 on the Female Infertility.

Objective: To study the clinical relation between serum CA125 and stromatosis in female infertility. Method: Radioimmunoassay was used to detect serum CA125 in 62 menstrualing patients with stromatosis. Results: The level of serum CA125 which was 83.2 ± 82.8 in menstrualing patients with stromatosis was higher than that with extensive adbesion in peivic hematoma which was 32.4 ±19. 5 ( P > 0. 01 ) and simple pelvic hematoma which was 29. 6 ± 12. 1 ( P < 0. 01 ). Pelvic hematoma would interfere with the diagnosis of stromatosis and the opsitive rate was low if only menstrualing serum CA125 was used. It was sugested that the sensitivity was 83.3% and specificity was 81.5% after the ratio serum CA125 in menstruation period/follicalar phase was higher than 2. 0, that can be used as an early diagnostic index to detect stromatosis.
