面部皮肤恶性肿瘤的手术治疗及其美学问题探讨

目的：探讨面部恶性肿瘤的手术治疗与相关美学问题。方法：1991年3月－2001年4月，收治面部皮肤恶性肿瘤40例，分别采用病灶扩大切除后直接缝合，全厚皮片分区移植，局部皮瓣和带蒂皮瓣等方法修复创面，并辅以放疗或化疗。结果：39例患者的移植皮片与皮瓣均100％成活，术区外形与功能良好，3例未接受放疗的基底细胞癌患者于术后9个月－1年1个月局部复发。结论：彻底切除面部恶性肿瘤并辅以放疗或化疗是预防术后复发的关键，适宜的修复方式有助于容貌恢复。

Operative treatment and aesthetic problem of facial skin malignant tumor

Objective: To study the operative treatment and aesthetic problem of facial skin malignant tumor. Methods: From March 1991 to April 2002, 40 patients with facial skin malignant tumor were treated in our department. Direct suture, full -thickness skin grafting, local skin flap and pedicle skin flap were used to repair wounds after extensive focus cleaning. Radiotherapy and chemotherapy were employed to be auxiliary treatment. Results:The skin grafts and various skin flaps in 39 patients survived. The appearance and function were very good. The recurrence appeared in 3 cases of basal cell carcinoma 9 months -13 months later post - operation , who didn't accept radiotherapy. Conclusions: It might be a key to remove the malignant tumor totally and then treat it with radiotherapy or chemotherapy to prevent the local recurrence. The suitable repair method might be helpful to recover the appearance.
