纵行带蒂岛状包皮瓣治疗尿道下裂44例报告

目的:总结近4年来应用纵行带蒂岛状包皮瓣一期尿道成形术治疗尿道下裂的临床经验,并评价其疗效.方法:本组44例患者中,阴茎型尿道下裂34例,阴茎阴囊型尿道下裂10例.均采用纵行带蒂岛状包皮瓣一期尿道成形术,平均重建尿道长度2.8 cm.结果:一期手术成功率90.9%(40/44例),尿瘘9.1%(4/44例).所有患者术后1个月复查1次,随访3个月以上,阴茎外观及排尿均良好.结论:纵行带蒂岛状包皮瓣一期尿道成形术对伴有阴茎下曲、尿道板发育不好的阴茎型及阴茎阴囊型尿道下裂患者治疗效果良好,并发症少.

One-stage repair for 44 cases of boy hypospadias with longitudinal preputial island flap

Objective:To evaluate the long-term results of 44 cases with hypospadias treated by one-stage Urethroplasty in the passed 4 years.Methods:44 cases of hypospadias treated form 2001 to 2005 were retrospectively analyzed.The age of the patients ranged from 1.8 years to 14 years(average age 4.4 years).Of them,34 were penis type,10 penoscrotal type.After release of chordee,one-stage repair of hypospadias with longitudinal preputial island flap has been achieved in 44 cases.The mean length of urethroplasty was 2.8 cm.Results:The successful rate in one-stage surgical repair was 90.9%(40/44).Postoperative complications included fistula(4).All patients were examined one month after operation.all the patient's penes were in good appearance and normal drainage during 3 months followed-up.44 cases were followed-up for 3 months-2 years.Conclusions:Longitudinal preputial island flap technique is a preferable method in the treatment of hypospadias,this flap and scrotal raphe flap for repairing of severe hypospadias was satisfactory.
