中老年2型糖尿病合并脂肪肝的临床分析

目的 :分析中老年 2型糖尿病合并脂肪肝的相关因素。方法 :检测中老年 2型糖尿病并发和不并发脂肪肝的患者空腹血糖 (FBG)、甘油三酯 (TG)、胆固醇 (TC)、高密度脂蛋白 -胆固醇 (HDL -C)、低高密度脂蛋白 -胆固醇 (LDL -C)、体重指数 (BMI)、腰 臀围比值 (WHR)等指标 ,同时检查是否合并其他糖尿病慢性并发症。结果 :脂肪肝组与非脂肪肝组比较 ,血TG增高、HDL -C降低 ,具有显著差异 (P <0 .0 1 ) ,且脂肪肝组腹型肥胖者及冠心病发生率高。结论 :中老年 2型糖尿病患者中脂肪肝的发生与高TG血症、腹型肥胖密切相关 ,常与冠心病合并存在 ,肥胖、脂质异常、胰岛素抵抗可能为脂肪肝、冠心病的共同病因 ,因此减肥、降脂、改善胰岛素抵抗 ,对防治糖尿病性脂肪肝有重要意义

CLINIC ANALYSES OF FATTY LIVER IN MIDDLE AGED AND SENILE PATIENTS WITH TYPE 2 DIABETES MELLITUS

Objective:To analyse the factors related to fatty liver in middle aged and senile patients with type 2 diabetes mellitus. Methods: The concentrations of FBG, TG,TC,HDL-C,LDL-C in serum, BMI and WHR were determined in 102 type 2 diabetic patients with or without fatty liver. Meanwhile, it was also determined whether they were combined with other chronic complications.Results: The concentration of TG in serum was higher,but that of HDL-C was lower significantly in patients with fatty liver than in control group(P<0.01). Abdominal adiposity and coronary heart disease in the patients with fatty liver occurred more often than those of the control group(P<0.01). Conclusions: The fatty liver in middle aged and senile patients with type 2 diabetes mellitus was significantly associated with hypertriglyceridemia and abdominal adiposity,and usually companied with coronary heart disease.Obesity,dyslipidemnia and insulin resistence may be the common etiology of the fatty liver and coronary heart disease. It is important to prevent and treat fatty liver by lowering weight,decreasing the concentration of plasma lipids and easing insulin resistence.
