摘    要:目的探讨神经导航联合术中实时超声在视觉功能区附近病变切除中的应用价值。方法回顾性分析2010年1月至2012年12月宁夏医科大学总医院神经导航联合术中超声在37例视觉功能区附近病变手术患者的临床资料。所有病人于术前进行导航序列影像采集,借助神经导航系统工作站,重建病灶与视辐射的三维图像位置,以此制定出个体化手术方案。术中联合实时超声避开视觉功能区切除肿瘤。结果肿瘤全切30例,7例大部分切除,34例术后视力视野与术前无明显变化,3例出现一过性加重,给予积极治疗后视力恢复术前水平。术后2周行KPS评分,100分36例,90分1例。结论对于视觉功能区病变神经导航可在术前制定个体化手术方案,结合术中实时超声扫描获得的实时信息引导颅内手术进程,提高了视觉功能区病变外科治疗的安全性和有效性,实现最大程度保护脑功能的同时最大范围切除肿瘤组织。

Abstract:Objective To evaluate the value of neuronavigation and traoperativeul trasound in the resection of lesions in or surrounding the occipital visual function area. Methods The clinical data in 37cases of gliomas in the eloquent brain regions treated by neuronavigation and intraoperative ultrasound in our hospital from January 2010 to december 2012 were retrospectively analyzed. Results Total lesion resection was achieved in 30 cases and subtotal resection in 7 cases. Visual functions improved or unchanged in 34 cases, temporary wors- ened in 3 cases. By postoperative routine treatment and nursing all were discharged from hospital with im- provement. Conclusion With the assistant of neuronavigation and the traoperativeul trasound was helpful to resect the lesion in or surrounding the occipital visual function, and was useful to decrease the side effect injury and enhance the life quality.
